COMMUNITY HEALTH: ENSURES ACCESS TO AFFORDABLE, QUALITY HEALTHCARE AND SUPPORT SERVICES THROUGH CASE MANAGEMENT, COACHING, CHRONIC DISEASE EDUCATION, AND TELEHEALTH SUPPORT. NORTHERN BROOME CARES SERVES OLDER ADULTS IN NORTHERN BROOME COUNTY TO SUPPORT HEALTH AGING. COMMUNITY HEALTH ALSO PROVIDES WORKFORCE DEVELOPMENT TRAINING ON SOCIAL DETERMINANTS OF HEALTH, RURAL CULTURAL COMPETENCE, AND HEALTH LITERACY.
RURAL HEALTH SERVICES CORPS- PROVIDE INDIVDUALS WITH OPPORTUNITIES TO SERVICE THEIR COMMUNITY FOR UP TO ONE YEAR. FOCUSES ON PLACEMENTS AND INITATIVES THAT SUPPORT COMMUNITY AND INDIVIDUAL HEALTH.
THE FOOD AND HEALTH NETWORK OF SOUTH CENTRAL NEW YORK IS A PROGRAM OF THE AGENCY THAT WORKS TO INCREASE THE CONSUMPTION OF HEALTHY, LOCALLY GROWN FOOD THROUGHOUT THE REGION. CURRENT COLLABORATIVE PROJECTS INCLUDE FARM TO SCHOOL, THE PRODUCE PRESCRIPTION PROGRAM, AND FOOD AS MEDICINE INITIATIVES. THE FOOD AND HEALTH NETWORK IS ACTIVELY INVOLVED IN IMPROVING FOOD SECURITY FOR VULNERABLE POPULATIONS AND INCREASING OUR KNOWLEDGE OF FOOD SYSTEMS THROUGH NEEDS ASSESSMENTS AND REGIONAL FOOD SYSTEM PLANNING.
GETTHERE- GETTHERE OPERATES A TRANSPORTATION INFORMATION AND ASSISTANCE CALL CENTER THAT WORKS TO HELP THOSE WITH TRANSPORTATION NEEDS TO FIND WORKABLE, AFFORDABLE SOLUTIONS. SERVING FIVE COUNTIES, GETTHERE PROVIDES TRANSPORTATION INFORMATION AND CASE MANAGEMENT SERVICES, FINANCIAL ASSISTANCE FOR ELIGIBLE INDIVIDUALS WHO NEED TRANSPORTATION TO HEALTHCARE APPOINTMENTS, TRANSPORTATION TO EMPLOYMENT SERVICES, AND TRAINING ON HOW TO USE PUBLIC TRANSPORTATION.
