THE MISSION OF APEX COMMUNITY CARE IS TO ADVOCATE FOR AND PROVIDE SERVICES TO PEOPLE LIVING WITH HIV/AIDS AND TO ACTIVELY PROMOTE AIDS EDUCATION & PREVENTION IN THE COMMUNITY.

RENTAL ASSISTANCE TO PERSONS WITH AIDS, SUPPORTIVE SERVICES ARE PROVIDED TO PROGRAM PARTICIPANTS THROUGH QUALIFIED STAFF. HOUSING INFORMATION SERVICES INCLUDING HOUSING SEARCH, ASSISTANCE, REFERRAL, ADVOCACY & COUNSELING. RESOURCE IDENTIFICATION THROUGH STAFF OUTREACH, RELATIONSHIP BUILDING & LOCATING HOUSING OPPORTUNITIES AND VACANCIES.
SUPPORTIVE SERVICES TO PERSONS WITH AIDS, WHICH INCLUDES CASE MANAGEMENT, MENTAL HEALTH COUNSELING, SUBSTANCE ABUSE COUNSELING, HOUSING REFERRALS, TRANSPORTATION, EMERGENCY FINANCIAL ASSISTANCE, LIFE SKILLS BUILDING, HIV COUNSELING & TESTING, YOUTH & COMMUNITY OUTREACH, AND NEEDLE EXCHANGE.
PRIMARY CARE FOR FOOD, UTILITIES & MEDICAL EXPENSES TO PERSONS WITH AIDS FOR PREVENTION & OUTREACH. PROGRAM STAFF WILL UTILIZE MULTIPLE TECHNIQUES TO DELIVER CULTURALLY AND LANGUAGE APPROPRIATE PREVENTION & RISK REDUCTION INFORMATION TO THE COMMUNITY. INDIVIDUAL LEVEL OUTREACH ACTIVITIES ARE BROUGHT TO WHERE PEOPLE ACTUALLY CONGREGATE. DISTRIBUTION OF HIV/AIDS PREVENTION MATERIALS, EDUCATIONAL LITERATURE & INFORMATION REGARDING COUNSELING & TESTING IS MADE AVAILABLE.
