IRIS HOUSE PROVIDES COMPREHENSIVE SERVICES AND ADVOCACY FOR WOMEN, FAMILIES, AND COMMUNITIES INFECTED WITH AND AFFECTED BY HIV/AIDS, WHILE SIMULTANEOUSLY PROVIDING PREVENTION AND EDUCATION SERVICES FOR OUR CLIENTS AND AT-RISK COMMUNITIES.

FOOD NUTRITION SERVICES THE FOOD AND NUTRITION DEPT. FNS IS DEDICATED TO IMPROVING THE HEALTH AND QUALITY OF LIFE OF OUR CLIENTS. THE FOLLOWING SERVICES ARE OFFERED TO OUR AGENCY AND COMMUNITY CLIENTS NUTRITION HEALTH EDUCATION, COOKING CLASSES, HEALTHY MOVEMENT FITNESS CLASSES, DIABETES MANAGEMENT WORKSHOPS, FRESH MARKET PRODUCE, PANTRY AND MEALS DISTRIBUTION, AS WELL AS REFERRALS TO SUPPORTIVE SERVICES. IN 2023, WE CONDUCTED NUTRITION HEALTH EDUCATION WITH 57 PEOPLE LIVING WITH HIV/AIDS PLWHAH, INCLUDING 251 INDIVIDUAL NUTRITION COUNSELING SESSIONS, 43 CASE CONFERENCES, 30 RESCREENING ASSESSMENTS, 305 FOOD VOUCHERS, 24 NUTRITION EDUCATION GROUP WORKSHOPS WITH 328 TOTAL PARTICIPANTS. WE ALSO CONDUCTED 30 COOKING CLASSES WITH A TOTAL OF 929 PARTICIPANTS, AND 34 HEALTHY MOVEMENT CLASSES WITH A TOTAL OF 960 PARTICIPANTS. ONE 1 CYCLE/COHORT OF DIABETES MANAGEMENT CLASS WAS CONDUCTED WITH 18 INDIVIDUALS WITH 15 GRADUATES. WE HOSTED 71 FRESH MARKETS AT FIVE MOBILE SITES INCLUDING OUR MAIN CENTRAL HARLEM OFFICE
HOUSING PROGRAMS IRIS HOUSE GOVERNS THREE HOUSING PROGRAMS SCATTER SITE HOUSING PROGRAM SSHP 86, HOUSING OPPORTUNITIES FOR PEOPLE LIVING WITH HIV/AIDS HOPWA, AND I.R.I.S. OUR HOUSING PROGRAMS ARE TAILORED TO MEET THE NEEDS OF ADULTS LIVING WITH HIV AND/OR LIVING WITH A MENTAL HEALTH DIAGNOSIS OR SUBSTANCE USE DISORDER. SERVICES PROVIDED TO CLIENTS INCLUDE CLIENT COLLABORATION ON GOAL/SERVICE PLANS, ASSESSMENTS, INTERNAL AND EXTERNAL REFERRALS INCLUDING BUT NOT LIMITED TO FOOD PANTRY, MENTAL HEALTH PROVIDERS, MEDICAL CARE, EDUCATIONAL ESL, ADULT LEARNING/HSE, AND LEGAL/IMMIGRATION RESOURCES. IN RESPONSE TO CLIENTS AGING WITH HIV WHO HAVE REQUESTED AEROBIC AND/OR DANCE CLASSES TO KEEP MOVING AND STAY HEALTHY, A MONTHLY DANCE CLASS WAS IMPLEMENTED AND HAS BEEN MADE ACCESSIBLE TO SSHP AND COMMUNITY MEMBERS. DURING 2023, IRIS HOUSE SUPPORTIVE HOUSING PROGRAMS PROVIDED HOUSING AND CASE MANAGEMENT SERVICES FOR 157 CLIENTS TOTALING
PREVENTION IN 2023, PREVENTION AND HEALTH EDUCATION SERVICES WERE PROVIDED IN NEW YORK CITY AND NEW JERSEY TO POPULATIONS WITH THE HIGHEST RATES OF NEW INFECTIONS AND COMMUNITIES FACING HEALTH DISPARITIES PARTICULARLY MEN AND WOMEN OF COLOR, MSM, YOUTH, LGBTQ COMMUNITY, AND MIGRANTS/IMMIGRANTS. THE PREVENTION DEPARTMENT OFFERED VARIOUS CDC EVIDENCE-BASED INTERVENTIONS SUCH AS SISTA, D-UP, WILLOW, SEEKING SAFETY, AND COMMUNITY PROMISE AS WELL AS LOVING OURSELVES, A HOMEGROWN INTERVENTION, AND LOVE YOUR LIFE A NATIONAL VIRTUAL SUPPORT GROUP FOR WOMEN LIVING WITH HIV. IN ADDITION TO GROUP LEVEL INTERVENTIONS, PREVENTION SERVICES INCLUDED RAPID HIV, HCV AND STI TESTING, EMOTIONAL WELLNESS GROUPS, HIV NAVIGATION, HEALTH EDUCATION WORKSHOPS, CONDOM DISTRIBUTION, VOCATIONAL SKILLS-BUILDING TRAININGS, SUBSTANCE USE TREATMENT, NON-MEDICAL CASE MANAGEMENT, COVID-19 SERVICES, PREP AWARENESS AND PHARMACY.
