TRANSITIONAL CARE: OUR ST. CLARE'S HOMES FOR CHILDREN PROVIDE A LOVING HOME TO CHILDREN IN NEED OF SPECIALIZED MEDICAL CARE IN TRANSITIONAL SETTING.
COMMUNITY OUTREACH SUPPORT PROGRAM: PROVIDE CASE MANAGEMENT, MENTAL HEALTH COUNSELING, SUBSTANCE ABUSE TREATMENT, PHARMACEUTICAL & FINANCIAL ASSISTANCE, & SUMMER CAMP PROGRAMS TO 350 FAMILIES LIVING WITH HIV/AIDS. ADDITIONALLY, ARDC PROVIDES 75 SCHOOL AGE CHILDREN AND ADOLESCENTS WITH OUT-OF-SCHOOL ACADEMIC AND PSYCHOSOCIAL SUPPORT.
HOUSING SERVICES: PROVIDE SHORT AND LONG TERM HOUSING, RENTAL AND UTILITY ASSISTANCE TO 150 FAMILIES LIVING WITH HIV/AIDS. ADDITIONALLY, ARFC PROVIDES TRANSITIONAL HOUSING AND SUPPORT SERVICES TO HIV POSITIVE WOMEN RE-ENTERING THE COMMUNITY FROM PRIOR INCARCERATION.
