DELIVER HEALTH EDUCATION PROMOTION TO THE TARGET POPULATION RELATED TO DISEASE PREVENTION SCREENINGS AND HEALTHY BEHAVIORS BY CONDUCTING COMMUNITY HEALTH FAIRS AND HELP CLIENTS AND THEIR FAMILIES OBTAIN ACCESS TO COMMUNITY SERVICES AND ADOPT HEALTHY BEHAVIORS.
