VNS HEALTH MLTC: THIS PROGRAM PROVIDES AN ARRAY OF HOME, COMMUNITY, AND FACILITY-BASED LONG-TERM CARE SERVICES TO ELIGIBLE ENROLLEES. MLTC PLUS PROGRAM PROVIDES BOTH MEDICARE AND MEDICAID BENEFITS TO THE ELIGIBLE ENROLLEES.
SELECTHEALTH FROM VNS HEALTH: THIS PROGRAM SERVES THEMEDICAID HIV SPECIAL NEEDS POPULATION. THIS HIGH NEEDS HIGH ACUITY POPULATION INCLUDES HOMELESS AND TRANSGENDER AS WELL. FOR THE LAST THREE CONSECUTIVE YEARS, SELECTHEALTH HAS HAD THE HIGHEST VIRAL LOAD SUPPRESSION RATE FOR THE HIV POSITIVE MEMBERSHIP AND CONTINUES TO PARTNER AS A RECIPIENT OF THE ENDING THE EPIDEMIC GRANT. VNS HEALTH EASYCARE PLUS IS A D-SNP DESIGNED TO OFFER FOCUSED CARE MANAGEMENT TO INDIVIDUALS THAT HAVE BOTH MEDICARE AND MEDICAID. VNS HEALTH EASYCARE IS AN HMO MEDICARE ADVANTAGE PRESCRIPTION DRUG (MAPD) PLAN DESIGNED TO MAKE MEDICAL CARE MORE AFFORDABLE. THE PLAN PROVIDES BOTH PART A & PART B COVERAGE TO ENROLLEES WHO ARE ELIGIBLE FOR MEDICARE BENEFITS.
