IT IS THE MISSION OF VISITING NURSE ASSOCIATION OF CENTRAL NEW YORK, INC. (DBA NASCENTIA HEALTH AT HOME) TO BE THE PREMIER HOME AND COMMUNITY-BASED CARE SYSTEM FOR THE REGIONS WE SERVE. VISITING NURSE ASSOCIATION OF CENTRAL NEW YORK, INC.'S GOAL OF MAINTAINING, RESTORING AND PROMOTING THE HEALTH AND INDEPENDENCE OF THOSE WE SERVE IS ACHIEVED BY UTILIZING OUR AGENCY AND COMMUNITY RESOURCES, BY WORKING COLLABORATIVELY, AND BY PROVIDING THE PROPER ENVIRONMENT FOR DEVELOPMENT AND RETENTION OF A CARING, COMPETENT STAFF.

SKILLED NURSING, THERAPY AND AIDE SERVICES PROVIDED TO PATIENTS IN THEIR HOMES, AS CERTIFIED AND REGULATED BY NEW YORK STATE DEPARTMENT OF HEALTH. IN THE YEAR 2022, APPROXIMATELY 86,000 VISITS WERE MADE TO APPROXIMATELY 4,300 PATIENTS. APPROXIMATELY 90% OF THE VISITS WERE FOR PATIENTS WHO QUALIFIED FOR MEDICARE, OR MEDICAID.
