OUR VISION IS TO PROVIDE PERSONALIZED SERVICES TO PEOPLE WITH DISABILITIES. OUR MISSION IS TO MAKE DREAMS HAPPEN. THROUGH RESIDENTIAL AND SUPPORTED LIVING ARRANGEMENTS, EMPLOYMENT, VOCATIONAL TRAINING, RECREATION, CLINICAL SERVICES, ADVOCACY AND INTERVENTION CCARC, INC. HAS IMPROVED THE LIVES OF INDIVIDUALS AND THEIR FAMILIES, AND ENHANCED THE COMMUNITY'S UNDERSTANDING ABOUT INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES.

Part 3 - Line 4a

COMMUNITY LIVING ARRANGEMENTS (11 HOMES). PROVIDE SERVICES FOCUSING ON THE INDIVIDUAL NEEDS OF EACH RESIDENT. CCARC WORKS WITH STATE AGENCY STAFF AND THE FAMILIES TO REALIZE THE DREAMS OF RESIDENTS AND DEVELOP COMPREHENSIVE PLANS TO MAKE IT HAPPEN. CCARC ALSO WORKS TO ENSURE QUALITY CARE FOR ALL RESIDENTS.


INDIVIDUALIZED HOME SUPPORT AND OTHER PROGRAMS


DAY SERVICES FOR APPROX 150 CLIENTS. THESE SERVICES INCLUDE DDS CONTRACT, IFS, SCHOOLS AND CLIENT FEES.


CRS PROGRAM - PROVIDE CONTINUOUS RESIDENTIAL SUPPORT AT 2 LOCATIONS.
