COMMUNITY COORDINATION - HELPS INDIVIDUALS WITH DISABILITIES WHO ARE AUTHORIZED FOR SERVICES BY THE STATE TO IDENTIFY, CHOOSE AND ATTAIN THEIR PREFERRED LIFESTYLE, AS PARTICIPATING MEMBERS OF THEIR COMMUNITIES, WITH EQUAL RIGHTS AND RESPONSIBILITIES. THE RESOURCE COORDINATOR HELPS TO LOCATE AND DEVELOP NECESSARY SUPPORTS AND SERVICES, ASSURING THAT EACH INDIVIDUAL'S NEEDS AND PREFERENCES ARE ADDRESSED, AND THEN MONITORS THE DELIVERY OF SERVICES TO ENSURE EFFICACY AND INDIVIDUAL SATISFACTION. THIS PROGRAM SERVED 1,951 INDIVIDUALS DURING THE CURRENT FISCAL YEAR.
