RHODE ISLAND PROGRAMS - THE ORGANIZATION PROVIDES A WIDE VARIETY OF SUPPORTS TO INDIVIDUALS WITH INTELLECTUAL AND DEVELOPMENTAL DISABILITIES IN OUR RHODE ISLAND LOCATION. PROGRAMS INCLUDE, DAY, COMMUNITY, FAMILY, RESIDENTIAL, SHARED LIVING, SELF DIRECTED, EMPLOYMENT AND AUTISM.
ADULT FOSTER CARE - THIS PROGRAM PROVIDES SERVICES TO MASSACHUSETTS RESIDENTS WITH INTELLECTUAL AND DEVELOPMENTAL DISABILITIES WHO NEED ASSISTANCE WITH ACTIVITIES OF DAILY LIVING IN ORDER FOR THEM TO SAFELY LIVE AT HOME. CASE MANAGERS AND NURSES FOR THIS PROGRAM OPERATE OUT OF OUR ATTLEBORO, FALL RIVER AND NEW BEDFORD OFFICES.
FAMILY SUPPORTS - THIS PROGRAM PROVIDES A VARIETY OF FAMILY SUPPORTS CENTER (FSC) SERVICES TO INDIVIDUALS WITH INTELLECTUAL AND DEVELOPMENTAL DISABILITIES, INCLUDING SUPPORTS RELATIVE TO INDIVIDUAL HOME, ADULT COMPANION, PCA SURROGACY, FINANCIAL ASSISTANCE, DESE, AUTISM, WINTER/SUMMER CAMP ACTIVITY, SATURDAY PROGRAM, ET CETERA. ADDITIONAL SUPPORTS PROVIDEDTHROUGH THE FSC ARE EXTENDED DAY, RESOURCE CENTER AND COMMUNITY LIVING SUPPORTS.
ADULT DAY HEALTH - THIS PROGRAM PROVIDES ADULT DAY HEALTH SERVICES TO THE ELDERLY AND CARING ENVIRONMENT FOR OLDER ADULTS WHOSE ABILITY TO FUNCTION INDEPENDENTLY HAS BEEN COMPROMISED BY MEDICAL, PHYSICAL, EMOTIONAL OR COGNITIVE HEALTH CONCERNS. SERVICES ARE CONDUCTED OUT OF THE WALTON STREET, ATTLEBORO LOCATION.DAY HABILITATION - THIS PROGRAM PROVIDES SERVICES TO INDIVIDUALS WITH INTELLECTUAL AND DEVELOPMENTAL DISABILITIES OUT OF THE FACILITY LOCATED IN MIDDLEBORO, MA. THE ORGANIZATION OFFERS DIRECT SUPPORT SERVICES THAT FOCUS ON SKILL BUILDING, INDEPENDENCE, AND COMMUNICATION OF EACH PARTICIPANT. NURSING SUPPORTS ARE ALSO PROVIDED FOR ESSENTIAL MEDICATION MANAGEMENT AND HEALTH EDUCATION. SERVICES RELATIVE TO PHYSICAL, OCCUPATIONAL AND SPEECH THERAPY ARE ALSO OFFERED.INDIVIDUALS SUPPORTS - THIS PROGRAM PROVIDES INDIVIDUAL SUPPORTS SERVICES TO PARTICIPANTS IN THE TAUNTON/ATTLEBORO COMMUNITIES WITH INTELLECTUAL AND DEVELOPMENTAL DISABILITIES. SUPPORTS PROVIDED ASSIST INDIVIDUALS WHO LIVE INDEPENDENTLY WITHIN THEIR COMMUNITY WITH TASKS SUCH AS BANKING, BUDGETING, MEDICAL TRANSPORTATION, RECERTIFICATION PAPERWORK FOR THEIR BENEFITS,GROCERY SHOPPING ETC.RESIDENTIAL SUPPORTS - THIS PROGRAM PROVIDES RESIDENTIAL SUPPORT SERVICES TO PARTICIPANTS IN THE TAUNTON/ATTLEBORO AND BROOKLINE COMMUNITIES WITH INTELLECTUAL AND DEVELOPMENTAL DISABILITIES. SUPPORTS ARE PROVIDED TO INDIVIDUALS WHO LIVE INDEPENDENTLY IN THEIR COMMUNITIES AND RECEIVE BETWEEN 15 TO 23 SUPPORTS HOURS PER DAY.GUARDIANSHIP SUPPORTS - THIS PROGRAM PROVIDES GUARDIANSHIP SUPPORT TO PARTICIPANTS WITH INTELLECTUAL AND DEVELOPMENTAL DISABILITIES. THE ORGANIZATION MONITORS ALL ASPECTS OF THE PARTICIPANTS LIFE, INCLUSIVE OF INDIVIDUAL NEEDS, SERVICES RECEIVED, AND MEDICAL ATTENTION. SERVICES ARE PROVIDED THROUGHOUT MASSACHUSETTS, INCLUDING FALL RIVER, NEW BEDFORD,ATTLEBORO, TAUNTON, MIDDLEBORO, WALTHAM, AND GREATER BOSTON.UMASS ACQUIRED BRAIN INJURY - THIS PROGRAM PROVIDES ASSISTANCE TO INDIVIDUALS WHO HAVE AN ACQUIRED BRAIN INJURY AFTER THE AGE OF 22. SUPPORT PROVIDED ARE FOR VARIOUS SERVICES FOR INDIVIDUALS PRESENTLY LIVING IN NURSING OR REHAB FACILITIES TO ASSIST THEM MOVING INTO THE COMMUNITY.OTHER SUPPORTS - THE ORGANIZATION ALSO PROVIDES SERVICES UNDER THE COMMUNITY DIRECTIONS, AGENCY WITH CHOICE SERVICES, TRUST MANAGEMENT, COMMUNITY EDUCATION AND ADVOCACY, REPRESENTATIVE PAYEE AND HOUSING PROGRAMS TO INDIVIDUALS WITH INTELLECTUAL AND DEVELOPMENTAL DISABILITIES THAT ARE DRIVEN BY THE SELF-DETERMINATION OF EACH INDIVIDUAL SUPPORTED.
