PRIMARY EXEMPT PURPOSE - IMPLEMENT INNOVATIVE PROGRAMS THAT IMPROVE ACCESS & DELIVERY OF CARE FOR PATIENTS WITH ACUTE ILLNESS: THE ORGANIZATIONS PRIMARY PURPOSE IS TO DEVELOP AND IMPLEMENT INNOVATIVE PROGRAMS TO IMPROVE ACCESS TO AND DELIVERY OF HEALTH CARE SERVICES FOR PATIENTS WITH ACUTE ILLNESS. THE ORGANIZATION WILL PURSUE A NUMBER OF PROGRAMS IN PURSUIT OF THESE GOALS INCLUDING DEVELOP TECHNOLOGY-BASED INNOVATIONS THAT WILL IMPROVE ACCESS TO MEDICAL RECORDS AMONG PROVIDERS OF SERVICES, IMPROVE COMMUNICATION AMONG PROVIDERS. THE ORGANIZATION WILL CONCENTRATE ITS EFFORTS ON "AT-RISK" POPULATIONS, SUCH AS THE FRAIL, ELDERLY, DISABLED, MENTALLY ILL AND PEDIATRIC POPULATIONS, ALL OF WHICH FREQUENTLY REQUIRE SPECIALIZED CARE TO EFFECTIVELY MANAGE ACUTE ILLNESSES. IN ORDER TO DO SO,THE ORGANIZATION INTENDS TO PARTNER WITH LOCAL AND NATIONAL LEADERS IN THE FIELDS OF GERIATRICS, PEDIATRICS AND OTHER SPECIALTIES IN ORDER TO INCORPORATE BEST HEALTHCARE.
