REFER TO SCHEDULE O FOR PROGRAM DESCRIPTIONONE COMMUNITY HEALTH SERVED OVER 13,768 INDIVIDUALS THROUGH 53,969 FACETOFACE ENCOUNTERS AND VIRTUAL VISITS IN 2021. RECOGNIZED AS A TIER 4 PATIENTCENTERED PRIMARY CARE HOME BY THE OREGON HEALTH AUTHORITY, OUR INTEGRATED PRIMARY CARE MODEL STRIVES TO PROVIDE AN OUTSTANDING PATIENT EXPERIENCE AND EXCEPTIONAL CLINICAL RESULTS. OUR MEDICAL, DENTAL, AND BEHAVIORAL HEALTH CARE TEAMS COMPRISED OF 96.95 FTE (FULL TIME EQUIVALENTS), 38.65 OF WHICH ARE LICENSED INDEPENDENT PROVIDERS. OCH'S PRIMARY CARE SERVICES INCLUDE: MATERNITY AND NEWBORN CARE (PRENATAL CARE, DELIVERIES INCLUDING CAESAREAN SECTIONS, POSTPARTUM AND NEONATAL CARE, AND PREVENTIVE DIAGNOSTICS). EVIDENCE BASED MEDICAL, DENTAL, AND BEHAVIORAL TREATMENT FOR ALL AGES. LABORATORY SERVICES, MEDICATION ASSISTANCE, IMMUNIZATIONS ALL AGES (INFANTS TO ADULTS), WELLCHILD EXAMS, PATIENT CASE MANAGEMENT, CHRONIC CARE MANAGEMENT, CANCER AND OTHER DISEASE SCREENINGS, DEVELOPMENTAL SCREENINGS FOR CHILDREN, AND FAMILY PLANNING SERVICES. OUTREACH AND EDUCATION (PROMOTORAS DE SALUD), INTERPRETATION, MEDICATION ASSISTED TREATMENT (MAT) FOR OPIOID ADDICTIONS, CASE MANAGEMENT AND PEER SUPPORT SERVICES, DIABETES AND MATERNITY CARE COORDINATION, AND ACUPUNCTURE. SPECIAL PROGRAMS IN AREAS SUCH AS DIABETES EDUCATION AND CASE MANAGEMENT, PERINATAL, PREVENTATIVE HEALTH, AND OTHERS HAVE BECOME PART OF THE FABRIC OF ONE COMMUNITY HEALTH. TWO OF OUR PHYSICIANS ARE DESIGNATED CIVIL SURGEONS AND PROVIDE MEDICAL EXAMINATIONS FOR IMMIGRATION APPLICANTS. OCH HAS ELIGIBILITY AND ENROLLMENT STAFF AT EACH OF ITS HEALTH CENTERS TO EDUCATE PATIENTS AND COMMUNITY MEMBERS ON AFFORDABLE HEALTH INSURANCE OPTIONS. WE PARTNER WITH PROVIDENCE HEALTH AND SERVICES IN PROVIDING AN INTEGRATED, RURAL, PRIMARY CARE RESIDENCY PROGRAM. ONE COMMUNITY HEALTH OPERATES THE ONLY SCHOOLBASED HEALTH CENTER IN HOOD RIVER COUNTY THAT SERVES ALL CHILDREN AND TEENS WHO RESIDE IN THE HOOD RIVER COUNTY SCHOOL DISTRICT. WE PARTNER WITH THE OREGON CHILD DEVELOPMENT COALITION (OCDC) TO PROVIDE PRIMARY CARE SERVICES TO THE CHILDREN OF MIGRANT AND SEASONAL FARMWORKERS DURING OREGON'S PEAK AGRICULTURAL MONTHS. WE PARTNER AND COORDINATE WITH OUTSTANDING LOCAL PROVIDERS FOR BEHAVIORAL HEALTH CARE, PHYSICAL THERAPY, HOSPITAL CARE, MASSAGE THERAPY, AND MANY OTHER SPECIALTY AND SUBSPECIALTY SERVICES ACCORDING TO PATIENT NEEDS. WE INCREASINGLY COORDINATE WITH COMMUNITY BASED SOCIAL SERVICE ORGANIZATIONS FOR NEEDS SUCH AS HOUSING, EMPLOYMENT, FOOD INSECURITY, ETC.WE ARE PROUD TO CONTINUE OUR MISSION OF ADVANCING HEALTH AND SOCIAL JUSTICE FOR ALL MEMBERS OF OUR COMMUNITY.NOTABLE ACCOMPLISHMENTS IN CALENDAR YEAR 2021: OCH PROVIDED MORE THAN 19,000 COVID19 VACCINES IN ITS FOURCOUNTY SERVICE AREA, THE SCALE OF WHICH WAS COMPARABLE TO MUCH LARGER HEALTH CENTERS AND HEALTH SYSTEMS. EXPANDED ITS OUTREACH PROGRAM AND ENGAGED ROUGHLY 2,000 MIGRANT AND SEASONAL FARMWORKERS AT MORE THAN 35 ORCHARDS, PACKINGHOUSES, AND OTHER COMMUNITY SETTINGS. PROVIDED: 1) EDUCATIONAL PRESENTATIONS ON HANDWASHING, MASKING, USE OF PERSONAL PROTECTIVE EQUIPMENT (PPE) AND OTHER INFECTION CONTROL MEASURES; 2) INFECTIOUS DISEASE EVALUATIONS OF OPERATIONS AT ORCHARDS AND PACKINGHOUSES; AND 3) ONSITE COVID19 TESTING FOR EMPLOYERS EXPERIENCING COVID19 EXPOSURE OR OUTBREAK. DISTRIBUTED MORE THAN 13,500 MASKS AND OTHER PPE TO FARMWORKERS, AS WELL AS ROUGHLY 17,700 COVID TEST KITS. ADDED ACUPUNCTURE AS A SERVICE OFFERING IN OUR HOOD RIVER CLINIC. REDESIGNED KEY PATIENT TOUCHPOINTS AND EXPERIENCES BY IMPLEMENTING ONLINE APPOINTMENT REQUESTS- OCH WAS ABLE TO PURCHASE AND OUTFIT A MOBILE MEDICAL UNIT TO BRING CARE DIRECTLY TO NATIVE COMMUNITIES IN THE COLUMBIA RIVER GORGE- RECEIVED A 2021 TOP WORKPLACE AWARD FROM THE OREGONIAN/OREGONLIVE.
