THE MISSION OF THE COALITION OF COMMUNITY HEALTH CLINICS IS TO IMPROVE HEALTHCARE FOR VULNERABLE POPULATIONS RESIDING IN THE PORTLAND-METRO AREA. WE STRENGTHEN COMMUNITY CLINICS, PROMOTE HEALTH EQUITY, AND FOSTER COLLABORATION ACROSS THE SAFETY-NET DELIVERY SYSTEM.

HEALTHCARE ENROLLMENT - THE COALITION PROVIDES OUTREACH AND ENROLLMENT ASSISTANCE TO SUPPORT THE TRANSITION OF UNINSURED TO THE OREGON HEALTH PLAN AND OTHER HEALTH INSURANCE PLANS. SERVICES INCLUDE OUTREACH TO BUILD AWARENESS ABOUT ENROLLMENT OPPORTUNITIES AND SERVICES, ASSISTANCE IN THE PROCESS OF ENROLLMENT, AND SUPPORT TO NAVIGATE AND CONNECT TO CARE.
INFORMATION AND REFERRAL - CCHC MANAGES PATIENT REFERRALS THROUGH THE INFORMATION AND REFERRAL PROGRAM. THE GOAL IS TO INCREASE ACCESS TO HEALTH SERVICES FOR UNINSURED, UNDERINSURED, MEDICAID AND OTHER PATIENTS WITHIN THE MEMBER CLINICS AS WELL AS FOR COMMUNITY MEMBERS NOT CURRENTLY CONNECTED TO CARE. THIS PROGRAM OFFERS INFORMATION AND REFERRALS TO PRIMARY CARE, URGENT CARE, ORAL HEALTH, INTEGRATIVE CARE, AND OTHER WELLNESS SERVICES FOR COMMUNITY MEMBERS VIA WEBSITE AND PHONE.
HEALTH AND HOUSING CCHC IS PART OF A MULTI-AGENCY MOBILE PERMANENT SUPPORTIVE HOUSING TEAM THAT PROVIDE RETENTION SERVICES FOR NEWLY HOUSED CLIENTS. THE PROGRAM SUPPORTS MEMBER CLINICS IN ADDRESSING HOUSING BARRIERS FOR THEIR CLIENTS AND WORKING WITH HOUSING PROVIDERS TO CONNECT NEWLY HOUSED COMMUNITY MEMBERS TO CLINIC SERVICES AND SUPPORTS.
