TO COLLABORATE WITH SCHOOL, COUNTY AND COMMUNITY YOUTH SERVING AGENCIES IN PROVIDING LIFE LONG LEARNING OPPORTUNITIES FOR YOUTH AND OTHERS WITH DISABILITIES.

Part 3 - Line 4a

DAY SERVICES: YEAR ROUND ADULTBRIDGE DAY SERVICES PROVIDE YEAR ROUND WEEKDAY ACTIVITIES OF DAILY LIVING AND LIFE SKILLS TRAINING FOR YOUNG ADULTS WITH DISABILITIES. BRIDGE DAY SERVICES SERVES 56 YOUNG ADULTS AND MEETS MONDAY THROUGH FRIDAY FROM 9 AM - 3 PM AT THE 651 BRAKKE DRIVE SITE THROUGH MARCH 13, 2020. AT THAT TIME THIS PROGRAM WAS SUSPENDED DUE TO THE COVID 19 PANDEMIC. PROGRAM RE-OPENED ON SEPTEMBER 1, 2021 WITH 17 CLIENTS. OUR HOPE IS TO GROW THE PROGRAM BACK TO ITS FORMER SIZE AS WE COME BACK FROM THE PANDEMIC. AT THE CLOSE OF THIS LAST FISCAL YEAR WE HAVE 28 CLIENTS ENROLLED IN THE PROGRAM.


COMMUNITY EMPLOYMENT SERVICES: BRIDGE COMMUNITY EMPLOYMENT SERVICES PROVIDE EMPLOYMENT SERVICES FOR 25 YOUNG ADULTS WITH DISABILITIES WHO ARE ENROLLED IN DEPARTMENT OF VOCATIONAL REHABILITATION (DVR) SERVICES.


BRIDGE TO HOME:BRIDGE FOR COMMUNITY LIFE LEASES DIRECT SUPPORT STAFF TO BRIDGE TO HOME SERVICES LLC. BRIDGE FOR COMMUNITY LIFE LEASES APPROXIMATELY 10-18 STAFF WHO PROVIDE COMPANION CARE FOR INDIVIDUALS WITH DISABILITIES IN THEIR HOMES.
