Part 3 - Line 4a
PLAN OF ARIZONA HAS TRIPLED THE NUMBER OF CLIENTS SERVED OVER THE PAST FIVE YEARS. WE CONTINUE TO EDUCATE FAMILIES WITH A DISABLED CHILD OR FAMILY MEMBER OF THE IMPORTANCE OF PLANNING FOR THE FUTURE OF THEIR LOVED ONE. WE DO THIS THROUGH OUTREACH AND WORKING WITH PROVIDER GROUPS SO FAMILIES KNOW THAT THERE ARE OPTIONS AVAILABLE FOR SETTING UP SPECIAL NEEDS TRUSTS AND PROVIDING CONTINUING CARE AFTER THE PRIMARY CARETAKER IS NO LONGER ABLE TO DO SO. ONCE FAMILIES HAVE SET UP A SPECIAL NEEDS TRUST WITH PLAN OF ARIZONA, WE CAN PROVIDE SERVICE COORDINATION, MONITORING, AND OTHER SUPPLEMENTAL NEEDS ABOVE AND BEYOND WHAT THEIR PUBLIC BENEFITS COVER.
