THE MISSION OF THE FOUNDATION IS TO ENABLE EACH INDIVIDUAL SERVED TO DISCOVER AND MEET HIS OR HER MAXIMUM POTENTIAL FOR INDEPENDENT, PRODUCTIVE LIVING, AND DEVELOPMENTAL GROWTH.

BLAKE CHILDREN & FAMILY SERVICES SERVED MORE THAN 2,500 CHILDREN ACROSS 10 COUNTIES OF SOUTHERN ARIZONA. FOCUSING ON THE FIRST 5 YEARS OF LIFE, THESE PROGRAMS ARE DESIGNED TO ASSIST NEW MOTHERS, SUPPORT FAMILIES IN TRACKING DEVELOPMENTAL MILESTONES AND IMPROVE THE QUALITY OF EARLY CHILDRHOOD EDUCATION IN SCHOOL AND AT HOME. BLAKE CHILDREN & FAMILY WAS ABLE TO OFFER BOTH VIRTUAL AND IN PERSON SERVICES TO CHILDREN & FAMILIES DURING THE COVID19 PANDEMIC RELATED SHUT DOWNS.
EASTERSEALS BLAKE BEHAVIORAL HEALTH PROVIDED COMPREHENSIVE BEHAVIORAL HEALTH TREATMENT FOR CHILDREN, ADULTS AND FAMILIES THROUGHOUT ARIZONA. THESE SERVICES INCLUDED INDIVIDUAL AND GROUP COUNSELING, PSYCHIATRIC SERVICES, SEMI-INDEPENDENT LIVING, RESIDENTIAL ADULT TREATMENT, AND APPLIED BEHAVIORAL INTERVENTIONS FOR ARIZONANS OF ALL AGES. THE BEHAVIORAL HEALTH PROGRAMS OF EASTERSEALS BLAKE FOUNDATION EMPHASIZE EASY ACCESS, COMPREHENSIVE CARE, EXCELLENT OUTCOMES THROUGH THE USE OF EVIDENCE BASED PRACTICES, AND EXCEPTIONAL SERVICE BUILT ON A CULTURE OF ENGAGEMENT AND WELLNESS. BLAKE BEHAVIORAL HEALTH WAS ABLE TO OFFER VIRTUAL SERVICES TO ENSURE CONTINUITY OF CARE THROUGHOUT THE PANDEMIC. BLAKE BEHAVIORAL HEALTH IS A DESIGNATED CENTER OF EXCELLENCE FOR PROVIDING BEHAVIORAL HEALTH SERVICES FOR CHILDREN BY ARIZONA COMPLETE HEALTH. DURING FY21, BLAKE BEHAVIORAL HEALTH PROVIDED AN AVERAGE OF 18,245 SERVICES PER MONTH.
EASTERSEALS BLAKE FOUNDATION'S BLAKE EMPLOYMENT AND COMMUNITY LIVING PROGRAMS PROVIDE SERVICES TO INDIVIDUALS WITH INTELLECTUAL AND DEVELOPMENTAL DISABILITIES IN 3 COUNTIES OF SOUTHERN ARIZONA.
