NCHP'S HOUSING FOR HEROES IS A VETERAN'S ADMINISTRATION PROJECT THAT ASSISTS VETERANS AND THEIR FAMILIES UNDER THE HOUSING FIRST PHILOSOPHY. THE SERVICES ARE DELIVERED VIA CASE MANAGEMENT PROCESS. DURING THE PERIOD OF JULY 1, 2021 TO JUNE 30, 2022 NCHP SERVED 539 PARTICIPANTS AND IMPARTED TEMPORARY FINANCIAL ASSISTANCE, TOTALING $843,554 FOR RENTS, SECURITY DEPOSITS, UTILITIES, UTILITY DEPOSITS, MOVING EXPENSES, EMERGENCY HOUSING ASSISTANCE, EMERGENCY FOOD ASSISTANCE, GENERAL HOUSING STABILITY ASSISTANCE, AND AUTO REPAIRS TO RETAIN EMPLOYMENT.
NATIONAL COMMUNITY HEALTH PARTNERS (NCHP) IMPLEMENTED PROYECTO PARA PREVENIR COVID-19 VACCINATION AND PREVENTION/EDUCATION CONTACTS PROGRAM ALONG THE ARIZONA-MEXICO BORDER. IN THE CATCHMENT AREA WERE 4 COUNTIES, PIMA, YUMA, COCHISE, AND SANTA CRUZ. THEY ALL HAD A CDC SOCIAL VULNERABILITY INDEX AT "HIGH LEVEL OF VULNERABILITY". NCHP EMPLOYED THE PROMOTORA MODEL IN PERFORMING OUTREACH, TRANSPORTATION, AND PREVENTION/EDUCATION CONTACTS, AND CONTRACTED WITH THE OTH MEDICAL GROUP TO VACCINATE ADULTS AND CHILDREN. NCHP PROPOSED TO HRSA TO VACCINATE 19,554 INDIVIDUALS AND PROVIDE 39,053 PREVENTION/EDUCATION CONTACTS. NCHP VACCINATED 29,340 INDIVIDUALS, AND HAD TOTAL PREVENTION/EDUCATION CONTACTS OF 57,071. OVERALL 9.8% OF THE VACCINATIONS WERE CHILDREN AND 90.2% WERE ADULTS WITH 71.8% VACCINATED IDENTIFYING AS HISPANIC.
