THE ORGANIZATION PROVIDES HEALTH NAVIGATION SERVICES AND COUNSELLING TO TRANSPLANT PATIENTS AT HOSPITALS IN THE SAN FRANCISCO BAY AREA WITH THE GOAL OF IMPROVING OUTCOMES FOR HEART TRANSPLANT PATIENTS. THE ORGANIZATION ASSIGNS PATIENTS A PHYSICIAN NAVIGATOR FROM THE TIME OF TRANSPLANT LISTING THROUGH THE FIRST MONTH FOLLOWING DISCHARGE FROM THE HOSPITAL. THE ORGANIZATION HELPS THE PATIENT THROUGH THE PROCESS BY PROVIDING ADVANCED CARE PLANNING, TRANSLATION SERVICES, SPIRITUAL NEEDS SUPPORT, SOCIAL WORK SUPPORT, AND HOUSING ANALYSIS AND SUPPORT.
