SKILLED NURSING AND REHABILITATION SERVICES:HCR MANORCARE, INC.'S SKILLED NURSING AND REHABILITATION CENTERSPROVIDE SHORT-TERM REHABILITATION AND SKILLED NURSING SERVICES FOR THOSE PATIENTS TRANSITIONING BETWEEN HOSPITAL AND HOME. AN INTERDISCIPLINARY TEAM APPROACH HELPS ENSURE THE PATIENT AND THEIR FAMILY ARE READY FOR A SAFE AND HEALTHY RETURN HOME. THE CENTERS ALSO PROVIDE LONG-TERM RESIDENTIAL CARE DESIGNED TO PROVIDE ASSISTANCE TO THOSE WHO ARE NO LONGER ABLE TO CARE FOR THEMSELVES DUE TO PHYSICAL LIMITATIONS, CHRONIC CONDITIONS OR CHANGES IN MENTAL CAPACITY. - SEE SCHEDULE O.
