TO IMPROVE THE QUALITY OF LIFE FOR THOSE INFECTED AND AFFECTED BY HIV/AIDS THROUGH CARE AND EDUCATION IN THE COMMUNITIES WE SERVE

Part 3 - Line 4a

RYAN WHITE PART B ENCOMPASSES BOTH MEDICAL OUTPATIENT AND SUPPORTIVE SERVICES. MEDICAL OUTPATIENT SERVICES INCLUDE PHYSICIAN CARE, PHYSICIAN REFERRALS, LAB WORK, AND OTHER ASSOCIATED PROFESSIONAL SERVICES. SUPPORTIVE SERVICES INCLUDE THE PROVISION OF CASE MANAGERS WHO ASSIST CLIENTS WITH SERVICES SUCH AS EMERGENCY HOUSING AND MEDICATION, MENTAL HEALTH, FOOD ASSISTANCE, TRANSPORTATION, ETC. ALL SERVICES OFFERED UNDER THIS GRANT ARE SUBJECT TO THE FINANCIAL AND RESIDENCY STATUS SET FORTH IN THE TERMS OF THE GRANT AGREEMENT.


NORTHLAND CARES IS THE CHOSEN FACILITY TO RUN THE HOPWA PROGRAM. HOPWA (HOUSING OPPORTUNITY FOR PEOPLE WITH AIDS/HIV) ASSISTS CLIENTS WHO ARE INFECTED WITH HIV WITH HOUSING, UTILITIES, EMPLOYMENT OPPORTUNITIES, AND GAINING SUSTAINABILITY. NORTHLAND CARES RECEIVES FUNDING FOR THIS PROGRAM THROUGH A GRANT AND IS RESPONSIBLE FOR HELPING ALL HIV CLIENTS IN NEED FOR THE COUNTIES OF YAVAPAI, APACHE, NAVAJO, COCONINO, AND MOHAVE.


THE 340B PHARMACY PROGRAM ALLOWS CLIENTS TO RECEIVE MEDICATIONS THROUGH A PHARMACY THAT PARTNERS WITH NORTHLAND CARES, THEREBY OFFERING PERSONALIZED SERVICES SUCH AS HOME DELIVERY, PHARMACY CONSULTS, AND REDUCED CO-PAYMENTS.


THE PREP PROGRAM, FUNDED THROUGH ADHS AND THE CDC WITH BOTH GRANTS AND 340B PROGRAM INCOME PROVIDES FOR HIV/STI/HEP PREVENTION ACTIVITIES, INCLUDING OUTREACH TO POPULATIONS AT RISK FOR HIV INFECTION, HIV TESTING, PRE-EXPOSURE PROPHYLAXIS AND SCREENING, AND REFERRAL FOR POST-EXPOSURE PROPHYLAXIS. THE PREP PROGRAM PARTNERS WITH THE YAVAPAI COUNTY HEALTH DEPARTMENT TO TEST VARIOUS TOWNS AND CITIES IN YAVAPAI COUNTY FOR HIV/HEP C/STI, PROVIDES EDUCATIONAL MATERIAL, PROPHYLAXIS AND PREVENTION SUPPLIES, AND NALOXONE TO HELP PREVENT OVERDOSES IN THE PWID POPULATION.
