SERVICES PROVIDED TO PEOPLE LIVING WITH HIV AND PEOPLE AT RISK FOR ACQUIRING HIV, PRIMARILY PEOPLE OF COLOR. HIV CARE SERVICES INCLUDE MEDICAL CASE MANAGEMENT, LINKAGE TO CARE AND TREATMENT, MENTAL HEALTH AND ADDICTION TREATMENT, PSYCHOSOCIAL SUPPORT, FINANCIAL ASSISTANCE, EMERGENCY HOUSING, TRANSPORTATION AND TRANSITIONAL SERVICES FOR PEOPLE LIVING WITH HIV IN JAILS AND PRISONS.
