THE PALM BEACH HABILITATION CENTER PROVIDES SUPPORTS AND SERVICES WHICH (CONTINUED ON SCH. O) ENABLE INDIVIDUALS WITH DISABILITIES TO REMAIN ACTIVE AND ENGAGED IN THEIR COMMUNITIES. DURING THIS FISCAL YEAR, 500 INDIVIDUALS WITH SIGNIFICANT DISABILITIES WERE SERVED BY THE CENTER. INDIVIDUALS IN OUR SUPPORTED EMPLOYMENT PROGRAM HAD A JOB RETENTION RATE OF 92% AND EARNED APPROXIMATELY $1.1 MILLION IN SALARIES THROUGH THEIR COMMUNITY-BASED JOBS. OTHER SUPPORT SERVICES INCLUDED: CASE MANAGEMENT, ADULT DAY TRAINING, AND RETIREMENT PROGRAMS FOR SENIORS, FOOD SERVICE TRAINING, COMPUTER SKILLS TRAINING, WORK TRAINING, AND RESIDENTIAL SERVICES WHICH INCLUDE GROUP HOMES AND SUPPORTED LIVING PROGRAMS. SERVICES ARE PROVIDED BASED ON THE UNIQUE NEEDS OF EACH PARTICIPANT. THE CENTER WAS ANNEXED LAST YEAR INTO THE VILLAGE OF PALM SPRINGS, FLORIDA. THE PHYSICAL LOCATION OF THE CENTER REMAINS THE SAME, BUT THE ADDRESS HAS CHANGES TO REFLECT THE ANNEXATION OF THE PROPERTY BY THE VILLAGE.

RESIDENTIAL - THE CENTER PROVIDES RESIDENTIAL SERVICES TO INDIVIDUALS WITH DISABILITIES THROUGH THREE GROUP HOMES, A TRANSITIONAL APARTMENT AND THE SUPPORTED LIVING PROGRAM. GROUP HOMES PROVIDE 24- HOUR SUPPORT TO RESIDENTS AND TRAINING TO INCREASE THEIR LIVING SKILLS. THE CENTER OPERATES JB'S RANCH, BABETTE WOLFF RESIDENCE, AND AMY'S HOUSE. JB'S RANCH IS A MEN'S GROUP HOME LOCATED IN LAKE WORTH WHICH SERVES 10 RESIDENTS. AN APARTMENT ON THE SAME PROPERTY PROVIDES THREE MEN THE OPPORTUNITY TO LIVE MORE INDEPENDENTLY, WHILE STILL HAVING THE SUPPORT OF THE GROUP HOME STAFF SHOULD ANY PROBLEMS OR ISSUES ARISE. BABBETTE WOLFFE RESIDENCE IS A CO-ED GROUP HOME LOCATED IN WEST PALM BEACH WHICH SERVES FIVE WOMEN AND TWO MEN. AMY'S HOUSE IS A WOMEN'S GROUP HOME LOCATED IN WEST PALM BEACH, FL (CONTINUED ON SCH. O) WHICH IS HOME TO SERVE SIX RESIDENTS. ALL THE GROUP HOMES PROVIDE A HOME-LIKE ENVIRONMENT FOR THE RESIDENTS AND ARE INTEGRATED INTO THE NEIGHBORHOOD.
COMMUNITY EMPLOYMENT: THE CENTER PROVIDES SUPPORTS AND SERVICES TO INDIVIDUALS WITH DISABILITIES TO ASSIST THEM IN OBTAINING AND MAINTAINING EMPLOYMENT. SERVICES INCLUDE PRE-EMPLOYMENT TRAINING, JOB PLACEMENT ASSISTANCE, JOB COACHING, AND FOLLOW-ALONG SERVICES TO INDIVIDUALS IN THE COMMUNITY. 8 INDIVIDUALS WERE PLACED INTO COMMUNITY-BASED EMPLOYMENT AND WERE PROVIDED WITH COACHING TO LEARN THE SPECIFIC REQUIREMENTS OF THEIR JOBS. 112 INDIVIDUALS ARE CURRENTLY RECEIVING LONG-TERM SUPPORTS TO ASSIST THEM IN MAINTAINING THEIR JOBS. INDIVIDUALS WHO HAVE THE SKILLS TO WORK IN THE COMMUNITY BUT NEED ADDITIONAL SUPERVISION AND SUPPORTS CAN PARTICIPATE IN THE CREW2U PROGRAM WHICH CONTRACTS WITH LOCAL BUSINESSES AND GOVERNMENT ENTITIES TO PERFORM SPECIFIC JOBS UNDER AN AGREED UPON (CONTINUED ON SCH. O) SCOPE OF SERVICES. SUPERVISORS ARE PROVIDED AS PART OF THIS CONTRACT TO ASSIST INDIVIDUALS TO REMAIN ON TASK AND PERFORM THE CONTRACT WORK UP TO THE EXPECTATION OF THE EMPLOYERS. SOME EMPLOYERS INCLUDE FLORIDA POWER AND LIGHT, PALM BEACH COUNTY PARKS AND RECREATION, HIGHWAY PATROL, CITY OF ATLANTIS, AND THE CITY OF WEST PALM BEACH.
TRAINING PROGRAMS - THE CENTER PROVIDES TRAINING PROGRAMS IN VOCATIONAL SKILLS AND ADULT DAILY LIVING SKILLS FOR INDIVIDUALS WITH DISABILITIES. VOCATIONAL SKILLS TRAINING INCLUDES FOOD SERVICE TRAINING, COMPUTER SKILLS TRAINING, PACKING, PACKAGING, AND MACHINE AND HAND ASSEMBLY. CONTRACTS ARE NEGOTIATED WITH COMPANIES TO BRING PRODUCTS AND JOBS INTO THE AGENCY FOR FULFILLMENT. THE WORK CONTRACTS ARE USED TO PROVIDE TRAINING AND EMPLOYMENT OPPORTUNITIES FOR PARTICIPANTS. 182 INDIVIDUALS PARTICIPATED IN THE WORK TRAINING PROGRAM AND EARNED OVER $260,000 IN WAGES FOR THE WORK PERFORMED. ADULT DAY TRAINING - DAILY LIVING SKILLS TRAINING OPPORTUNITIES ARE PROVIDED TO INDIVIDUALS THROUGH GENERAL PROGRAMS (CONTINUED ON SCH. O) AND SPECIFIC PROGRAMS TO ADDRESS UNIQUE NEEDS OF INDIVIDUALS, SUCH AS THOSE WITH PHYSICAL LIMITATIONS AND SENIORS. THE PERSONAL ARTS AND CREATIVE EXPRESSION PROGRAM (PACE) HAS BEEN DEVELOPED TO INTEGRATE THE ARTS INTO ALL ASPECTS OF INDIVIDUALS' TRAINING OPPORTUNITIES.
