MISSION:

THE PALM BEACH HABILITATION CENTER PROVIDES SUPPORTS AND SERVICES WHICH (CONTINUED ON SCH. O) ENABLE INDIVIDUALS WITH DISABILITIES TO REMAIN ACTIVE AND ENGAGED IN THEIR COMMUNITIES. DURING THIS FISCAL YEAR, 500 INDIVIDUALS WITH SIGNIFICANT DISABILITIES WERE SERVED BY THE CENTER. INDIVIDUALS IN OUR SUPPORTED EMPLOYMENT PROGRAM HAD A JOB RETENTION RATE OF 92% AND EARNED APPROXIMATELY $1.1 MILLION IN SALARIES THROUGH THEIR COMMUNITY-BASED JOBS. OTHER SUPPORT SERVICES INCLUDED: CASE MANAGEMENT, ADULT DAY TRAINING, AND RETIREMENT PROGRAMS FOR SENIORS, FOOD SERVICE TRAINING, COMPUTER SKILLS TRAINING, WORK TRAINING, AND RESIDENTIAL SERVICES WHICH INCLUDE GROUP HOMES AND SUPPORTED LIVING PROGRAMS. SERVICES ARE PROVIDED BASED ON THE UNIQUE NEEDS OF EACH PARTICIPANT. THE CENTER WAS ANNEXED LAST YEAR INTO THE VILLAGE OF PALM SPRINGS, FLORIDA. THE PHYSICAL LOCATION OF THE CENTER REMAINS THE SAME, BUT THE ADDRESS HAS CHANGES TO REFLECT THE ANNEXATION OF THE PROPERTY BY THE VILLAGE.

MISSION:

THE ORGANIZATION IS A FLORIDA CHARTER SCHOOL AS PROVIDED FOR AND BY FLORIDA STATUTES. THE SCHOOL IS AUTHORIZED BY & UNDER CONTRACT WITH THE PALM BEACH COUNTY FLORIDA SCHOOL DISTRICT. THE SCHOOL PROVIDES EDUCATION FOR HIGH SCHOOL STUDENTS WITH AN EMPHASIS ON ARTS AND THEATER.

MISSION:

THE FOP JIM FOGLEMAN LODGE #50 FOUNDATION INCORPORATED IS ORGANIZED EXCLUSIVELY FOR CHARITABLE, RELIGIOUS, EDUCATIONAL, AND SCIENTIFIC PURPOSES, INCLUDING, FOR SUCH PURPOSES, THE MAKING OF DISTRIBUTIONS TO ORGANIZATIONS THAT QUALIFY AS EXEMPT ORGANIZATIONS DESCRIBED UNDER SECTION 501(C)(3) OF THE INTERNAL REVENUE CODE, OR CORRESPONDING SECTION OF ANY FUTURE TAX CODE.

MISSION:

THE ORGANIZATION PROVIDES THE FOLLOWING SERVICES FOR HOMELESS UNITED STATES VETERANS: (1) TRANSITIONAL HOUSING (2) ASSISTANCE WITH FOOD AND HOUSING, AS WELL AS, EMOTIONAL AND PSYCHOLOGICAL SUPPORT AND AFTERCARE SERVICES. (3) HALFWAY HOME FOR HOMELESS VETERANS RECENTLY RELEASED FROM THE WEST PALM BEACH MEDICAL CENTER. (4) SUPPORTIVE SERVICES FOR VETERAN FAMILIES (SSVF), PROVIDING A RANGE OF SUPPORTIVE SERVICES TO ELIGIBLE VETERAN FAMILIES THAT ARE DESIGNED TO PROMOTE HOUSING STABILITY AMOUNG VERY-LOW INCOME VETERAN FAMILIES WHO RESIDE IN OR ARE TRANSITIONING TO PERMANENT HOUSING, AS WELL AS VETERANS AT RISK OF HOMELESSNESS.

MISSION:

FLORIDA OUTREACH CENTER FOR THE BLIND IS ORGANIZED AND OPERATED TO RAISE FUNDS IN ORDER TO PROVIDE VISUALLY IMPAIRED PEOPLE, TRAINING IN VARIOUS INDEPENDENT LIVING SKILLS IN A FULL-SERVCE TRAINING FACILITY

The goal of this organization is to provide multipurpose centers or services that meet the needs and interests of a particular target population.

AUDIOLOGY WITH A HEART, INC IS A NON-PROFIT AGENCY WHOSE MISSION IT IS TO PROVIDE AFFORDABLE HEARING AIDS, OTHER ASSISTIVE DEVICES AND AUDIOLOGY SERVICES TO THE GENERAL PUBLIC.

The goal of this organization is to provide, on an outpatient basis, a combination of treatment and education services to restore maximum functioning, a sense of well-being and a personally satisfying level of independence for individuals who have temporary or permanent disabilities.

The goal of this organization are the membership organizations that are engaged in promoting the interests of the veterans or active or retired military personnel who are their members.

RESCUE AND REHABILITATE DOGS FROM PALM BEACH COUNTY AND SURRONDING AREAS OUT OF SHELTERS AND THOSE THAT ARE HOMELESS AS WELL AS TO BRING AWARENESS TO THE IMPORT

PROVIDE FOOD AND SHELTER FOR APPROXIMATELEY 35 WOMEN AND CHILDREN. PROVIDE COUNSELING AND GROUP SUPPORT FOR APPOXIMATELY 35 PERSONS.
