WE DO THIS THROUGH EDUCATION AND COMMUNITY AWARENESS OF RELATED MATERNAL AND INFANT HEALTH ISSUES, TARGETED OUTREACH TO AND CASE MANAGEMENT OF AT-RISK FAMILIES.
