THE MISSION OF HOSPICE CLEVELAND COUNTY IS TO RELIEVE SUFFERING AND IMPROVE THE QUALITY AND DIGNITY OF LIFE THROUGH COMPASSIONATE HOSPICE CARE FOR THOSE AT THE END OF LIFE, PALLIATIVE CARE FOR THOSE WITH ADVANCED ILLNESS, AND THROUGH COMMUNITY EDUCATION.

HOSPICE CLEVELAND COUNTY PROVIDES HOSPICE CARE IN A HOMECARE SETTING THAT CONSISTS OF COMPREHENSIVE END OF LIFE CARE FOR TERMINALLY ILL PATIENTS AND THEIR FAMILIES DURING THE FINAL STAGES OF ILLNESS. HOME HOSPICE CARE IS PROVIDED IN A PATIENTS PRIVATE RESIDENCE, NURSING FACILITY, OR HOSPITAL. AS A NOT-FOR-PROFIT ORGANIZATION, HOME HOSPICE CARE IS PROVIDED REGARDLESS OF A PATIENTS INSURANCE STATUS OR ABILITY TO PAY. THESE HOME HOSPICE CARE SERVICES INCLUDE AN INTERDISPLINARY TEAM THAT IS MADE UP OF NURSING CARE, PHYSICIAN VISITS, SOCIAL WORK AND CHAPLAIN SERVICES. IN ADDITION, ANCILLARY SERVICES AND BEREAVEMENT SUPPORT ARE PROVIDED TO THE PATIENT'S FAMILY AFTER THE PATIENT'S DEATH.
HOSPICE CLEVELAND COUNTY OPERATES TWO HOSPICE HOUSES THAT PROVIDE HOSPICE INPATIENT AND RESIDENTIAL CARE FOR TERMINALLY ILL PATIENTS DURING THE FINAL STAGES OF ILLNESS. THE WENDOVER HOSPICE HOUSE HAS 16 BEDS AND THE TESTA FAMILY HOSPICE HOUSE HAS 8 BEDS. THESE HOSPICE HOUSE CARE SERVICES INCLUDE AN INTERDISCIPLINARY TEAM THAT IS MADE UP OF NURSING CARE, PHYSICIAN VISITS, SOCIAL WORK, AND CHAPLAIN SERVICES. IN ADDITION, ANCILLARY SERVICES AND BEREAVEMENT SUPPORT ARE PROVIDED TO THE PATIENT'S FAMILIES AFTER THE PATIENT'S DEATH.
