RESIDENTIAL SERVICES: TO PROVIDE COMMUNITY-BASED LIVING AND SUPPORTS IN TRADITIONAL HOUSING FOR INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES. PROVIDED GROUP HOME SERVICES IN NORMALIZED COMMUNITY HOMES FOR 12 INDIVIDUALS INCLUDING BASIC SKILLS OF DAILY LIVING, NUTRITION, AND OTHER SUPPORT SERVICES.
COMMUNITY EMPLOYMENT: EVALUATION, JOB DEVLOPMENT, JOB TRAINING, AND EMPLOYMENT FOLLOW-ALONG SERVICES FOR INDIVIDUALS WITH DISABILITIES. 346 PEOPLE WERE SERVED IN EVALUATION, JOB DEVELOPMENT, JOB TRAINING, AND SHORT & LONG TERM EMPLOYMENT FOLLOW-ALONG SERVICES. 65 INDIVIDUALS WERE PLACED IN JOBS.
INNOVATIONS: PROVIDES INDIVIDUALIZED ONE-ON-ONE COMMUNITY BASED SUPPORTS FOR INDIVIDUALS WHO WOULD OTHERWISE NEED INSTITUTIONAL SUPPORT. THE PROGRAM HAS PROVIDED UP TO 20 HOURS PER WEEK OF ONE-ON-ONE COMMUNITY BASED SUPPORT FOR 15 INDIVIDUALS WITH DISABILITIES.
