THESE SUPPORTS AND SERVICES INCLUDE: SUPPORTED LIVING SUPERVISED LIVING ALTERNATIVE FAMILY LIVING. COMMUNITY ACTIVITIES SUPPORTED EMPLOYMENT MORE THAN 50 INDIVIDUALS ARE SERVED ON AN ANNUAL BASIS.
