MAKING ALL THE CHILDREN HEALTHY (MATCH) PROVIDES MEDICAL CASE MANAGEMENT AND HEALTH CARE COORDINATION FOR ALL CHILDREN CONNECTED WITH THE BALTIMORE CITY DEPARTMENT OF SOCIAL SERVICES FOSTER CARE SYSTEM. PROGRAM STAFF ENSURE ENROLLMENT IN MARYLAND'S MEDICAL ASSISTANCE INSURANCE PROGRAM AND COORDINATE MEDICAL, BEHAVIORAL HEALTH, AND DENTAL EXAMS. THE MATCH TEAM PROVIDES SPECIALIZED CARE COORDINATION FOR CHILDREN WITH COMPLEX BEHAVIORAL AND PHYSICAL HEALTH NEEDS, AND SUPPORTS THE HEALTH NEEDS FOR PREGNANT AND/OR PARENTING YOUTH IN FOSTER CARE.
OTHER PROGRAM SERVICES TO FURTHER THE ORGANIZATION'S EXEMPT PURPOSE BY CONNECTING RESIDENTS TO PUBLIC HEALTH CARE COVERAGE AND HELPS THEM NAVIGATE SERVICES EFFECTIVELY. THE MARYLAND ESTABLISHMENT GRANT FOR THE AFFORDABLE CARE ACT PROVIDES MARYLAND RESIDENTS ASSISTANCE IN NAVIGATING HEALTH PLAN OPTIONS AND ENROLLMENT IN A HEALTH CARE PLAN, AS WELL AS DETERMINING ANY POTENTIAL SUBSIDY AMOUNTS.
THE ELIGIBILITY DEPARTMENT ASSISTS CONSUMERS IN BALTIMORE CITY WITH APPLYING FOR AND RENEWING MEDICAID HEALTH COVERAGE. THE PROGRAM SERVES FAMILIES, PREGNANT AND POST-PARTUM INDIVIDUALS, CHILDREN, AND NEWBORNS. THIS LEGACY PROGRAM HAS BEEN ASSISTING CONSUMERS SINCE 1997.
