MGH COMMUNITY HEALTH, INC., IS A FREE-STANDING HOME HEALTH AGENCY AND CONSISTED OF THREE DIVISIONS, MGH HOME HEALTH, MGH PERSONAL CARE AND THE DIABETES & NUTRITION CENTER. MGH COMMUNITY HEALTH PROVIDED FREE HEALTH SERVICES TO RESIDENTS OF MONTGOMERY COUNTY IN THE FORM OF COMMUNITY LECTURES, BLOOD PRESSURE CLINICS, FLU SHOTS AND TELE-HEALTH MONITORING SERVICES. MGH HOME HEALTH IS A MEDICARE CERTIFIED, STATE LICENSED HOME HEALTH AGENCY THAT PROVIDED PHYSICIAN ORDERED CARE TO PATIENTS IN THEIR PLACE OF RESIDENCE. SERVICES INCLUDE MEDICAL/SURGICAL NURSING, MENTAL HEALTH NURSING, INFUSION NURSING, PHYSICAL THERAPY, OCCUPATIONAL THERAPY, SPEECH-LANGUAGE PATHOLOGY, SOCIAL WORK, AND HOME HEALTH AIDE SERVICES. TELE-HEALTH MONITORING SERVICES WERE PROVIDED AS AN INTEGRAL PART OF OUR PATIENT CARE PLAN. MGH PERSONAL CARE WAS A STATE LICENSED, RESIDENTIAL SERVICE AGENCY PROVIDING HOME CARE AIDE AND SKILLED NURSING SERVICES TO CLIENTS IN THEIR HOMES TO FACILITATE INDEPENDENCE AND QUALITY OF LIFE FOR EACH CLIENT. THE DIABETES AND NUTRITION CENTER WAS A MEDICARE CERTIFIED DIABETES AND NUTRITION EDUCATION PROGRAM RECOGNIZED BY THE AMERICAN DIABETES ASSOCIATION. SERVICES INCLUDED INDIVIDUAL AND GROUP EDUCATION AND COUNSELING IN DIABETES SELF-CARE AND NUTRITION COUNSELING TO CLIENTS WITH CARDIAC DISEASE, CANCER AND OTHER DIABETES MODALITIES. IN A PREVIOUS YEAR, ALL OF THESE ACTIVITIES WERE TRANSFERRED TO VNA, INC.
