DAY PROGRAM: THE ORGANIZATION PROVIDES A DAY PROGRAM FOR OLDER ADULTS WITH DISABILITIES. SERVICES PROVIDED INCLUDE REGISTERED NURSE OVERSIGHT, MEDICATION ADMINISTRATION, SIX HOURS OF THERAPEUTIC ACTIVITIES, AGE APPROPRIATE MEALS AND SNACKS, ASSISTED TRANSPORTATION AS NEEDED, AND SOCIAL WORKER OVERSIGHT AND CASE MANAGEMENT, AS NEEDED.
ASSISTED LIVING: THE ORGANIZATION PROVIDED ASSISTED LIVING AND OVERNIGHT RESPITE CARE. SERVICES PROVIDED INCLUDE MEDICATION ADMINISTRATION, ASSISTANCE WITH ACTIVITIES OF DAILY LIVING AS NEEDED, AGE APPROPRIATE MEALS AND SNACKS, LAUNDRY AND HOUSEKEEPING SERVICES.
