OUTREACH: COORDINATORS CONDUCT OUTREACH AND HEALTH INSURANCE LITERACY EFFORTS IN HARD-TO-REACH, LOW-INCOME AND MINORITY COMMUNITIES, OFFERING INFORMATION IN MULTIPLE LANGUAGES.ENROLLMENT: CLIENTS ARE ASSISTED WITH ENTERING THEIR INFORMATION IN A DATABASE TO DETERMINE ELIGIBILITY FOR 18 HEALTH AND SOCIAL SERVICE PROGRAMS. CLIENTS CAN LEARN ABOUT AND ENROLL IN A VARIETY OF PROGRAMS IN A SINGLE VISIT.CASE MANAGEMENT: THIS PROCESS PROVIDES REGULAR CONTACT FOR UP TO ONE YEAR TO HELP RESIDENTS VERIFY ONGOING ELIGIBILITY, FIND MEDICAL PROFESSIONALS, MAKE APPOINTMENTS, MITIGATE ANY BARRIERS TO CARE AND RENEW COVERAGE FOR THE NEXT YEAR.
