CARE TEAM SUPPORT - THE PURPOSE OF THE CARE TEAM IS TO PARTNER WITH OUR SINGLE MOM CLIENTS TO WALK ALONGSIDE THEM, HELPING TO REMOVE THE ROADBLOCKS THAT CAN ARISE AS THEY WORK TO BRING CHANGE TO THEIR FAMILIES. EACH CLIENT HAS THE OPPORTUNITY TO MEET WITH A CARE TEAM COORDINATOR WHO IS TRAINED TO GET TO THE DEEPER CHALLENGES OUR MOMS FACE. THEY WORK WITH EACH CLIENT TO BUILD OUT A CARE PLAN THAT ASSISTS IN GETTING FROM POINT A TO POINT B, WHATEVER THEIR INDIVIDUAL POINTS MAY BE. ONCE A CARE PLAN IS CREATED, CLIENTS ARE THEN PAIRED WITH A CARE TEAM COORDINATOR WHO WILL EQUIP AND EMPOWER OUR MOMS TO ACHIEVE THE GOALS SET.
FINANCIAL AID PROGRAM: THIS PROGRAM CAN BE APPLIED FOR BY THE CLIENTS IN OUR PROGRAM. WE DETERMINE WHERE THEY AR WITHIN FINANCIAL LITERACY AND THEN WE DECIDE HOW WE CAN SUPPORT THEM THROUGH OUR FINANCIAL AID PROGRAM, OR DIRECT THEM TO OTHER ORGANIZATIONS THAT WE PARTNER WITH. DURING 2022, WE PROVIDED MONETARY SUPPORT FOR ITEMS SUCHA S PAST DUE RENT, UTILITIES, HOME AND CAR REPAORS, AND OTHER EVERYDAY NECESSITIES. THESE ARE ALSO CLIENTS THAT HAD THEIR OWN CARE PLAN WITH US.
PROFESSIONAL SERVICES: WE ASSISTED OUR CLIENTS AS A COMMUNITY RESOURCE LIAISON AIDING OUR CLIENTS TO RESOURCES IN THE COMMUNITY. WE ALSO PARTNERED WITH ORGANIZATIONS TO OFFER FINANCIAL BUDGTING WORKSHOPS.
