Describe the Organization's Mission:

Part 3 - Line 1

JUBILEE HOME COLLABORATES WITH JUSTICE INVOLVED FOLK TO FOSTER BELONGING, PROMOTE HEALING, AND BUILD WELLNESS INTO OUR COMMUNITY THROUGH QUALITY, LOW BARRIER, SUPPORTIVE HOUSING.

Describe the Organization's Program Activity:

Part 3 - Line 4a

IN 2023, JUBILEE HOME PROVIDED HOUSING FOR 23 INDIVIDUALS AND ENGAGED IN SUPPORTIVE SERVICES WITH OVER 100 COMMUNITY MEMBERS OUTSIDE OF OUR RESIDENTS. OF OUR 23 RESIDENTS HOUSED, 10 LIVED IN OUR PERMANENT HOUSING AND 13 RESIDED IN OUR TRANSITIONAL HOUSING SPACE. OF THE 13 TRANSITIONAL RESIDENTS, 8 SUCCESSFULLY MOVED TO INDEPENDENT LIVING, 2 STILL RESIDED AT JUBILEE HOME AS OF JANUARY 1, 2024, 1 NEEDED TO MOVE TO A SUBSTANCE USE DISORDER TREATMENT PROGRAM, AND 1 WAS RE-INCARCERATED. NINE OF THIRTEEN RESIDENTS CARRY THE BURDEN OF A SUBSTANCE USE DISORDER, NINE OF THIRTEEN LIVE WITH A BEHAVIORAL HEALTH DIAGNOSIS, AND SIX OF THIRTEEN HOLD THE CO-OCCURRING OF BOTH. AN INDIVIDUAL EXITING INCARCERATION IS NOW 50 TIMES MORE LIKELY TO DIE IN THE FIRST TWO WEEKS OUT OF PRISON OR JAIL FROM OVERDOSE THAN THE GENERAL POPULATION, BUT IN FIVE YEARS OF OPERATION, JUBILEE HOME HAS HAD NO FATAL OVERDOSES. IN 2023, 9 OF 13 RESIDENTS IN OUR TRANSITIONAL PROGRAM FOUND GAINFUL EMPLOYMENT, TWO LEARNED SKILLS TO HELP THEM MANAGE THEIR FIXED DISABILITY INCOME, AND 2 INDIVIDUALS DID NOT FIND EMPLOYMENT PRIOR TO THEIR EXITS FROM JUBILEE HOME. NEITHER OF THE TWO UNEMPLOYED RESIDENTS STAYED AT JUBILEE HOME LONGER THAN A MONTH. ALL THIRTEEN RESIDENTS IN OUR TRANSITIONAL PROGRAM WERE CONNECTED TO PRIMARY CARE PHYSICIANS, MENTAL HEALTH SUPPORTS, FINANCIAL LITERACY, AND PROVIDED A MINIMUM OF 10 HOURS OF DIRECT PEER SUPPORT PER WEEK. ALONG WITH INTENSIVE CASE MANAGEMENT SUPPORT, JUBILEE HOME ALSO MANAGES A COMMUNITY GARDEN THAT PROVIDES BEAUTY AND LIFE TO OUR COMMUNITY AND PRODUCED NEARLY A TON OF PRODUCE THROUGHOUT THE YEAR.
