THE ALZHEIMER'S EARLY DETECTION PROJECT IS A MEMORY SCREENING ASSESSMENT, PREVENTION, AND TREATMENT PROGRAM FOR SENIORS WITH RISK FACTORS OF COGNITIVE MEMORY DECLINE. THE PROGRAM PROVIDES FREE COGNITIVE MEMORY SCREENING EXAMS, DISEASE EDUCATION, CAREGIVER EDUCATION AND REFERRALS OF AT-RISK PATIENTS TO PHYSICIANS FOR FOLLOW-UP ASSESSMENT.
A CAREGIVER SUPPORT GROUP AND CRISIS MANAGEMENT SUPPORT PROGRAM TO CREATE AND PREVENT BURNOUT, REDUCE LEVELS OF STRAIN AND DEPRESSION, AND INCREASE CAREGIVER CAPACITY WITH THE PRIMARY INTENT ON DELIVERING INDIVIDUAL CRISIS MANAGEMENT SUPPORT. SUPPORT INCLUDES IMMEDIATE HELP ON ADRD BEHAVIORAL INTERVENTIONS, NEGLECT AND ABUSE, WANDERING, THEFT, REHOUSING, DEPRESSION, BURNOUT, REFERRALS, AND OTHER COMPLICATIONS THAT ARISE AND THREATEN THE TREATMENT OF ADRD.
ALZHEIMER'S CARE PROJECT IS AN IN-HOME ASSESSMENT, TREATMENT & MANAGEMENT PROGRAM FOR PEOPLE LIVING WITH ALZHEIMER'S DISEASE OR RELATED DEMENTIA THAT ARE HOME BOUND.
