PROVIDING THE BEST POSSIBLE HEALTH CARE OFTEN GOES BEYOND AN EXAM AND A PRESCRIPTION REFILL. OFTEN, ESPECIALLY AS WE AGE, WE DEVELOP MORE COMPLEX NEEDS THAT MIGHT INVOLVE MULTIPLE CONDITIONS, MANY PRESCRIPTIONS, NUMEROUS PROVIDERS, VISITS TO THE HOSPITAL AND EVEN SITUATIONS OUTSIDE OF THE HEALTH SYSTEM, SUCH AS FINANCIAL STRESSES, THAT CAN IMPACT OUR ABILITY TO GET WELL AND STAY WELL. THAT'S WHY MEDICARE IS WORKING WITH MANY DOCTORS, HOSPITALS AND OTHER HEALTH CARE PROVIDERS TO PROMOTE BETTER, MORE COORDINATED HEALTH CARE. TO ALLOW THIS COORDINATION, HARTFORD HEALTHCARE MEDICAL GROUP HAS CREATED A TEAM-BASED APPROACH TO MEDICINE KNOWN AS AN ACCOUNTABLE CARE ORGANIZATION.
