BLUE CIRCLE HEALTH IS A COLLABORATIVE TYPE 1 DIABETES (T1D) NON-PROFIT DEDICATED TO EMPOWERING AND IMPROVING THE LIVES OF PEOPLE WITH T1D. OUR UNIQUE, VIRTUAL APPROACH LEVERAGES PARTNERSHIPS WITH PATIENTS, FAMILIES, MEDICAL PROFESSIONALS, AND COMMUNITY-BASED ORGANIZATIONS TO REMOVE BARRIERS TO CARE AND PROMOTE UNDERSTANDING OF THIS COMPLEX DISEASE. TO ENSURE THAT PATIENTS LIVING WITH T1D CONNECT TO THE SERVICES THEY NEED TO ACHIEVE THEIR HEALTH GOALS, WE OFFER DIABETES MEDICAL CARE, SUPPORTIVE COUNSELING, INSURANCE NAVIGATION, DISEASE EDUCATION, PEER COACHING, ACCESS TO SUPPLIES AND MEDICATION, AND REFERRALS TO COMMUNITY RESOURCES. WE TAKE AN INNOVATIVE, WHOLE-PERSON APPROACH TO T1D CARE THAT AIMS TO EMPOWER THE PEOPLE WE SERVE. WE PARTNER WITH OUR PATIENTS TO IDENTIFY AND ACHIEVE SPECIFIC T1D GOALS WHILE CONNECTING THEM TO A SUPPORTIVE COMMUNITY OF PEERS AND PROFESSIONALS TO ENSURE THEIR ONGOING SUCCESS. WE PUT PATIENTS FIRST. WE CURRENTLY SUPPORT ADULTS AND THEIR FAMILIES LIVING WITH T1D IN FLORIDA. WE PLAN TO GROW TO REACH PEOPLE THROUGHOUT THE COUNTRY. THESE EARLY YEAR PROGRAM EXPENSES STEM FROM CORE ORGANIZATIONAL COMPONENTS RELATED TO LAUNCHING OUR INITIAL SERVICES, INCLUDING THE FOUNDATIONAL HUMAN CAPITAL AND INFORMATION TECHNOLOGY INFRASTRUCTURE THAT WILL EVOLVE WITH THE GROWING NEEDS OF BLUE CIRCLE HEALTH TO ACHIEVE THE ORGANIZATION'S MISSION. IN JULY 2023, WE LAUNCHED OUR FULL COMPLEMENT OF SERVICES. OUR FREE VIRTUAL CARE PROGRAM PROVIDES PEOPLE WHO LIVE WITH T1D THE SUPPORT, SERVICES AND RESOURCES NEEDED TO HELP MANAGE THIS DIFFICULT DISEASE. EACH PERSON RECEIVES AN INDIVIDUAL CARE PLAN DESIGNED TO HELP ADDRESS THEIR UNIQUE T1D-RELATED NEEDS. CARE PLANS ARE DESIGNED TO LAST ABOUT SIX MONTHS AND MAY INCLUDE SOME OR ALL OF THE SERVICES LISTED ABOVE. WHEN THE INDIVIDUAL CARE PLAN IS COMPLETED, PEOPLE CAN REMAIN CONNECTED TO THE BLUE CIRCLE HEALTH T1D COMMUNITY FOR ONGOING SUPPORT AND CAN RE-ENROLL IF NEEDED.
