HARBORPATH IS A SOUTH CAROLINA NONPROFIT CORPORATION THAT WILL CONDUCT PHARMACEUTICAL PROGRAMS CONSISTENT WITH THE EXEMPT PURPOSES LISTED IN SECTION 501(C)(3) OF THE INTERNAL REVENUE CODE. HARBORPATH WILL SERVE QUALIFIED LOW-INCOME UNINSURED INDIVIDUALS WITH LIFE-THREATENING AND/OR CHRONIC CONDITIONS SUCH AS HIV/AIDS, HEPATITIS, AND OPIOID SUBSTANCE USE DISORDERS THROUGHOUT THE UNITED STATES. HARBORPATH WILL PROVIDE A SEAMLESS SOLUTION TO DELIVER THERAPEUTIC MEDICATION AT NO COST - THROUGH A SIMPLIFIED ONE-STOP-SHOPPING PROCESS. HARBORPATH ENABLES THOSE PROVIDING CARE AND TREATMENT FOR HIV/AIDS, HEPATITIS, AND OPIOID SUBSTANCE USE DISORDERS TO FILL THE MEDICATION GAP FOR THE UNINSURED. THROUGH HARBORPATH'S PARTNERSHIPS WITH INDUSTRY LEADERS, THE ORGANIZATION WILL DIRECTLY IMPACT THE LIVES OF PATIENTS SUFFERING FROM HIV/AIDS, HEPATITIS, AND OPIOID SUBSTANCE USE DISORDERS AT THE COMMUNITY AND NATIONAL LEVELS. EXAMPLES OF THE PROGRAMS THAT WILL BE CONDUCTED BY HARBORPATH INCLUDE THE FOLLOWING: (1) MANAGE PATIENT ASSISTANCE PROGRAM ELIGIBILITY PROCESS ON BEHALF OF PHARMACEUTICAL COMPANIES, (2) DELIVER MEDICATIONS TO PATIENTS/PRESCRIBERS/CLIENTS. RECONNECT PATIENTS TO STATE FUNDED DRUG ASSISTANCE PROGRAMS WHEN POSSIBLE, AND (3) PROVIDE INFORMATION TO PEOPLE WHO ARE INELIGIBLE FOR HARBORPATH SERVICES REGARDING HOW THEY CAN ACCESS TREATMENT.
