COMMUNITY CARE NETWORK, INC. IS A HOSPITAL OWNED PHYSICIAN NETWORK OFFERING A BROAD RANGE OF SERVICES AND SPECIALTIES TO CARE FOR ITS PATIENTS WITHOUT REGARD TO THEIR ABILITY TO PAY. COMMUNITY CARE NETWORK, INC. PROVIDES LEADING-EDGE CARE TO PATIENTS IN NORTHWEST INDIANA. THROUGH ITS DEDICATION TO QUALITY AND INNOVATION, COMMUNITY CARE NETWORK, INC. CONTINUES IN ITS PURSUIT OF IMPROVING THE HEALTH OF ITS PATIENTS AND COMMUNITY. AS A DIRECT RESULT OF THE HOSPITALS LATEST CHNA IMPLEMENTATION PLAN, COMMUNITY CARE NETWORK, INC.
