NAVIGATOR - OUTREACH AND ENROLLMENT ASSISTANCE TO CONSUMERS ON MISSOURI MARKETPLACE INSURANCE AND PREMIUM TAX CREDITS (AFFORDABLE CARE ACT)
BENEFITS ENROLLMENT CENTER - EVIDENCE PERSON-CENTERED STRATEGIES IN A COORDINATED, COMMUNITY-WIDE SYSTEM TO FIND AND COMPLETE APPLICATIONS ON BEHALF OF MEDICARE BENEFICIARIES, BOTH SENIORS AND PERSONS WITH DISABILITIES, WHO HAVE LIMITED INCOME AND RESOURCES IN AVAILABLE BENEFITS WITH PRIMARY FOCUS BEING IN SUBMITTING APPLICATIONS FOR THE FOLLOWING BENEFIT PROGRAMS: MEDICARE PART D EXTRA HELP (LOW INCOME SUBSIDY, LIS), MEDICARE SAVINGS PROGRAMS (MSP), MEDICAID, SUPPLEMENTAL NUTRITION ASSISTANCE PROGRAM (SNAP), AND LOW-INCOME HOME ENERGY ASSISTANCE (LIHEAP).
FUTURE OF AGING - THE OVERALL GOAL OF THE PROJECT IS TO CREATE AN INTERDISCIPLINARY NETWORK OF SERVICE PROVIDERS AND OTHER ORGANIZATIONS THAT WILL UNIFY IN ADVOCATING FOR PROGRAMS, POLICIES, AND SYSTEMS CHANGE THAT SUPPORT HEALTH, INDEPENDENCE, AND DIGNITY FOR ALL OLDER ADULTS AND THEIR FAMILIES IN MISSOURI.
