THE MISSION OF MENTAL HEALTH RESOURCES IS TO FOSTER HOPE, HEALTH AND RECOVERY FOR PEOPLE AFFECTED BY MENTAL ILLNESS AND SUBSTANCE USE DISORDER (SUD).

TARGETED CASE MANAGEMENT (TCM) TEAMS PROVIDED PERSON-CENTERED SUPPORT TO ADULTS AND CHILDREN WHO HAVE BEEN DIAGNOSED WITH A SERIOUS AND PERSISTENT MENTAL ILLNESS. MHR PROVIDES PERSONALIZED SERVICE FOR INDIVIDUALS IN THE COMMUNITY WHO MAY HAVE CO-OCCURRING MEDICAL, SOCIAL AND SUBSTANCE ABUSE CHALLENGES. TCM CASE MANAGERS HELP CLIENTS GAIN ACCESS TO SERVICES NEEDED TO MEET THEIR MENTAL HEALTH NEEDS, ACHIEVE RECOVERY, AND LIVE INDEPENDENTLY IN THE COMMUNITY. SERVICES ARE PROVIDED BY SKILLED MENTAL HEALTH PRACTITIONERS WHO ARE SUPPORTED BY A TEAM LEADER AND A CLINICAL SUPERVISOR. CASE MANAGERS WORK CLOSELY WITH EACH INDIVIDUAL TO CREATE A TREATMENT PLAN, BASED ON ASSESSMENT OF FUNCTIONING, NEEDS AND STRENGTHS. WE MEET WITH INDIVIDUALS 1-4 TIMES PER MONTH IN THEIR HOME OR IN THE COMMUNITY. IN 2023, 2,550 CLIENTS WERE SERVED.
IN 2023, MHR SERVED 13,195 INDIVIDUALS. WHILE THE MAJORITY OF OUR CLIENTS RESIDE IN HENNEPIN, RAMSEY AND DAKOTA COUNTIES, WE PROVIDE SERVICES IN 22 COUNTIES THROUGHOUT MINNESOTA. THE VAST MAJORITY OF MHR CLIENTS ARE LOW- INCOME OR LIVING BELOW THE FEDERAL POVERTY LEVEL. OVER 96% OF OUR CLIENTS ARE ENROLLED IN PUBLIC PROGRAMS; 2% ARE UNINSURED AND COVERED THROUGH COUNTY CONTRACTS; AND 2% ARE ENROLLED WITH A COMMERCIAL INSURANCE PLAN (INCLUDING MEDICARE COMMERCIAL PLANS). IN ADDITION TO THE PROGRAMS LISTED PREVIOUSLY, OUR CURRENT PROGRAMS AND SERVICES INCLUDE: SUPPORTIVE HOUSING PROGRAMS: HOUSING IS AMONG THE GREATEST NEEDS IN THE POPULATION WE SERVE. THE CONCEPTS "SUPPORTIVE HOUSING- AND "HOUSING FIRST" MEAN THE CLIENTS ARE MORE LIKELY TO SUCCEED IN THEIR RECOVERY AND ABILITY TO LIVE IN THE COMMUNITY IF THEY CAN SECURE HOUSING (AND SUBSIDIES) WITH SUPPORTIVE SERVICES AVAILABLE, OFTEN ON SITE. IN 2023, 40 CLIENTS WERE SERVED. SPECIAL NEEDS BASIC CARE (SNBC): SNBC PROVIDES CARE COORDINATION TO CONSUMERS WHO HAVE A DIAGNOSIS OF A MENTAL HEALTH, PHYSICAL HEALTH OR DEVELOPMENTAL DISABILITY. CARE COORDINATOR (CC) CONDUCTS AN ASSESSMENT, THEN ESTABLISHES, CARRIES OUT, AND MAINTAINS A CARE PLAN TO MEET EACH MEMBER'S NEEDS TO MAINTAIN HIS OR HER HEALTH WHILE HELPING THEM NAVIGATE COMPLEX SOCIAL SERVICE AND HEALTH CARE DELIVERY SYSTEMS. IN 2023, 8,981 CLIENTS WERE SERVED. OUTPATIENT SUBSTANCE USE DISORDER (SUD): MINNESOTA ALTERNATIVES (A PROGRAM WITHIN MHR)PROVIDES ADULT OUTPATIENT MENTAL HEALTH AND/OR SUBSTANCE USE TREATMENT AND CONSULTATION SERVICES FOCUSING ON ENGAGEMENT, SKILL DEVELOPMENT,PERSONALIZED INTERVENTIONS, NEUROSCIENCE AND HEALING TRAUMA. THE PROGRAMUSES MANY STATE OF THE ART INTERVENTIONS INCLUDING MINDFULNESS MEDITATION,IMAGERY, MOTIVATIONAL ENHANCEMENTS, COGNITIVE BEHAVIORAL STRATEGIES, ANDMULTIPLE TRAMUA THERAPIES. THE PROGRAM SPECIALIZES IN SERVING PEOPLE WHO EXPERIENCE BOTH SUBSTANCE USE ISSUES AND MENTAL HEALTH PROBLEMS AND IS ENROLLED WITH THE DEPARTMENT OF HUMAN SERVICES AS A PROVIDER OF CO-OCCURRING SERVICES. HOWEVER, A MENTAL ILLNESS IS NOT REQUIRED TO BE ELIGIBLE FOR SERVICES. IN 2023, 132 CLIENTS WERE SERVED. METRO INTENSIVE TREATMENT TEAM (MITT): MITT IS A MOBILE, COMMUNITY-BASED, COLLABORATIVE MODEL OF TREATMENT DESINGED TO DELIVER VARIABLE LEVELS OF SERVICE INTENSITY TO CLIENTS WITHIN THEIR COMMUNITIES AND THEIR HOMES. SERVICES ARE INDIVIDUALIZED AND COMPREHENSIVE WITH SHORT-TERM STRATEGIC INTERVENTIONS INTENDED TO STABLIZE CLIENTS AND PREVENT OR SHORTEN HOSPITAL STAYS, AVOID COMMITMENT TO LONG-TERM HOSPITALIZATION AND PREVENT THE LOSS OF EMPLOYMENT, HOUSING AND PERSONAL RELATIONSHIPS. IN 2023, 322 CLIENTS WERE SERVED. SEWARD COMMUNITY SUPPORT PROGRAM (CSP): THE CSP CONSISTS OF A TEAM OF MENTAL HEALTH PRACTITIONERS THAT WORK WITH ADULTS DIAGNOSED WITH SERIOUS AND PERSISTENT MENTAL ILLNESS RESIDING INDEPENDENTLY IN THE COMMUNITY. TO INCREASE SYMPTOM STABILITY AND TO SUPPORT THE MEMBER'S ABILITY TO REMAIN IN THE COMMUNITY, THE TEAM ASSESSES THE INDIVIDUAL'S NEEDS AND THEN PROVIDES OR COORDINATES SERVICES TO MEET THOSE NEEDS. THESE SERVICES CAN BE PROVIDED AT OUR SEWARD DROP-IN CENTER OR IN THE MEMBER'S HOME. IN 2023, 218 CLIENTS WERE SERVED. ADDITIONALLY, IN 2023, MHR PROVIDES THE FOLLOWING SUPPORT SERVICES FOR CLIENTS. O MENTAL HEALTH OUTREACH CLINIC (MHOC): THE CLINIC IS AN ESSENTIAL COMMUNITY PROVIDER THAT SERVES HIGH-RISK, SPECIAL NEEDS, AND UNDERSERVED INDIVIDUALS. THE CLINIC SPECIALIZES IN PROVIDING IN-HOME AND TELEHEALTH THERAPY TO INDIVIDUALS AND FAMILIES REGARDLESS OF THEIR FINANCIAL LIMITATIONS. O MHR HEALTH & WELLNESS: OUR HEALTH AND WELLNESS DEPARTMENT HELPS TO PROVIDE TRAINING, SKILL DEVELOPMENT AND RESOURCES TO OUR MHR PROGRAMS SO THAT THEY CAN ASSIST IN MEETING THE HEALTH NEEDS OF THE PEOPLE WE SERVE. SOME OF THE CURRENT PROGRAMMING INCLUDES TOBACCO CESSATION INITIATIVE, HEALTHY EATING INITIATIVE, AND A COMPREHENSIVE DENTAL PROGRAM TO CONNECT CLIENTS TO BEST PRACTICES IN DENTAL CARE. MHR CONDUCTS AN ANNUAL STATISFACTION SURVEY. IN 2023, 96% OF SURVEY PARTICIPANTS INDICATED THEY WERE VERY SATISFIED OR SATISFIED WITH THE SERVICES THEY HAVE RECEIVED FROM MHR. 92% OF SURVEY PARTICIPANTS INDICATED THEIR LIFE IS BETTER SINCE RECEIVING SERVICES FROM MHR.
HOUSING VOUCHER PROGRAM: MHR ADMINISTERS HOUSING VOUCHERS IN THE 7-COUNTY METRO AREA FOR INDIVIDUALS LIVING WITH MENTAL ILLNESS, CHEMICAL DEPENDENCY AND OTHER DISABILITIES. THESE RENTAL ASSISTANCE SUBSIDIES ARE FROM A VARIETY OF SOURCES INCLUDING HUD SHP (RAMSEY, ANOKA, DAKOTA, AND HENNEPIN COUNTIES), BRIDGES, SECTION 8, HOUSING TRUST FUND AND RAMSEY SUPPORTIVE HOUSING VOUCHER PROGRAM. IN 2023, 1,224 CLIENTS WERE SERVED.
ASSERTIVE COMMUNITY TREATMENT (ACT) IS A SERVICE MODEL PROVIDED BY COMMUNITY- BASED, MOBILE MENTAL HEALTH TREATMENT TEAMS. THE ACT TEAM APPROACH IS DESIGNED TO PROVIDE COMPREHENSIVE PSYCHIATRIC TREATMENT, REHABILITATION, AND SUPPORT TO PERSONS WITH SERIOUS AND PERSISTENT MENTAL ILLNESS, OR PERSONALITY DISORDERS WITH SEVERE FUNCTIONAL IMPAIRMENTS, TO LIVE INDEPENDENTLY IN THE COMMUNITY. PERSONS SERVED BY ACT OFTEN HAVE CO- EXISTING PROBLEMS SUCH AS HOMELESSNESS, SUBSTANCE ABUSE, FREQUENT HOSPITALIZATION, AND/OR INVOLVEMENT WITH THE JUDICIAL SYSTEM. PEOPLE TYPICALLY SERVED BY ACT TEAMS OFTEN HAVE NEEDS THAT HAVE NOT BEEN EFFECTIVELY ADDRESSED BY TRADITIONAL, NON-INTENSIVE SERVICES. TEAM MEMBERS WORK WITH PEOPLE IN THEIR HOMES, WORK SETTING, OR PLACES IN THE COMMUNITY WHERE ADDITIONAL SUPPORT MIGHT BE NEEDED. IN 2023, 193 CLIENTS WERE SERVED.
