CHRONIC DISEASE PREVENTION:THIS PROJECT PROVIDES CONTINUED IMPLEMENTATION OF CHRONIC DISEASE COORDINATING NETWORK DESIGNED TO REDUCE THE IMPACT THAT CHRONIC DISEASE CONTINUES TO PLAY IN POOR HEALTH OUTCOMES AND RISING HEALTH CARE EXPENSES ACROSS THE REGION. THE PROJECT IS A COLLABORATIVE EFFORT OF THE REGIONAL NORTHWEST MICHIGAN CHRONIC DISEASE PREVENTION COALITION, WHICH SERVES AS THE FOUNDATION FOR THE NETWORK, AND THE LOCAL HEALTH COALITIONS ACROSS THE REGION, WHICH SERVE AS THE LOCAL IMPLEMENTATION PARTNERS. THIS PROJECT FOCUSES ON STRATEGIES THAT LINK DIRECTLY TO THE DHD#10 CHIP SPECIFICALLY TO FOCUS AREAS: PROMOTION OF HEALTHY LIFESTYLES AND BEHAVIORS AND CREATION AND MAINTENANCE OF HEALTHY COMMUNITIES TARGETING AT-RISK POPULATIONS AND MEDICALLY UNDERSERVED POPULATIONS.
MEDICAL ACCESS PROGRAM:INCLUDES AFFORDABLE HEALTHCARE ACT OUTREACH AND ENROLLMENT FOR AT-RISK POPULATIONS. FREE INFLUENZA VACCINATION FOR AT-RISK POPULATIONS WHO DO NOT HAVE HEALTHCARE COVERAGE. COVERAGE OF FAMILY PLANNING SERVICES FOR INDIVIDUALS WHO ARE UNINSURED OR UNDER-INSURED FOR CONTRACEPTIVE COVERAGE.PREMIUM SUBSIDY FOR INDIVIDUALS WHOSE PREMIUMS FOR HEALTHCARE COVERAGE EXCEED 9.5% OF THEIR INCOME.
COMMUNITY HEALTH IMPROVEMENT PLAN (CHIP)IMPLEMENTATION:THIS FUNDING PROVIDES SUPPORT OF COLLABORATIVE ACTIONS TO MOVE IMPLEMENTATION OF THE CHIP NOT COVERED BY THE CHRONIC DISEASE PREVENTION IMPLEMENTATION PROJECT. STRATEGIES INCLUDE: PARTICIPATION IN COUNTY HEALTH COALITIONS; DEVELOP COALITION WORK PLANS TARGETING IDENTIFIED GOALS, OBJECTIVES AND STRATEGIES THAT ALIGNMENT WITH THE CHIP; FOCUS ON MATERNAL SMOKING PRIORITY AREA OF THE CHIP INCLUDING TOBACCO TREATMENT, PROVIDER EDUCATION & SOCIAL MEDIA/MARKETING PLAN; FOCUS ON REDUCING UNINTENDED & TEEN PREGNANCY RATES; FOCUS ON INCREASING IMMUNIZATION RATES THROUGH IMPLEMENTATION OF A MEDIA/MARKETING/OUTREACH CAMPAIGN; WORK WILL FOCUS ON STRATEGIES THAT TARGET THE UNDER-INSURED AND MOST VULNERABLE POPULATIONS; ASSURE ANNUAL EVALUATION AND REPORTING ON CURRENT CHIP; COLLABORATE WITH THE 8 HOSPITAL PARTNERS ON COMMUNITY HEALTH IMPROVEMENT PRIORITIES.
