MUSKEGON COMMUNITY HEALTH PROJECT (MCHP), IN PARTNERSHIP WITH MERCY HEALTH MUSKEGON, IS AN INCLUSIVE, COMMUNITY-BASED, DECISION-MAKING NON-PROFIT AGENCY COMMITTED TO IMPROVING THE HEALTH OF THE COMMUNITY AND THE DELIVERY OF HEALTH CARE IN MUSKEGON COUNTY AND THE SURROUNDING WEST MICHIGAN REGION. MCHP ALSO COORDINATES THE COMMUNITY BENEFIT PROGRAMMING FOR MERCY HEALTH MUSKEGON BY OVERSEEING THE COMMUNITY HEALTH NEEDS ASSESSMENT PROCESS. SERVICES INCLUDE COORDINATING ACCESS TO HEALTH COVERAGE, DIABETES MANAGEMENT, EYE CARE, ASSISTANCE WITH MEDICATIONS, AND LUNG-FUNCTION TESTING.THE PROJECT IS ONE OF THE OLDEST AND MOST SUCCESSFUL COMMUNITY-BASED COLLABORATIVE NETWORKS OF ITS TYPE IN THE NATION. MCHP HAS WORKED WITH COMMUNITY HEALTH ORGANIZATIONS AND INDIVIDUALS TO PIONEER SOME OF THE MOST INNOVATIVE SOLUTIONS TO HEALTH ACCESS PROBLEMS IN THE NATION. MCHP'S EFFORT TO CREATE AFFORDABLE HEALTH COVERAGE HAS BEEN REPLICATED IN SEVERAL STATES. MCHP'S COMMUNITY PROGRAMS REDUCE INAPPROPRIATE USE OF ANTIBIOTICS, EXPAND DENTAL CARE, AND MANAGE DIABETES, AND HAVE RECEIVED NOTEWORTHY PRAISE FROM OUTSIDE THE COMMUNITY.
