WESTSHORE HEALTH NETWORK CONTRACTS ON BEHALF OF ITS PHYSICIAN, HOSPITAL AND ANCILLARY PROVIDERS WITH LOCAL, STATEWIDE AND FEDERAL PAYERS TO PROVIDE NETWORK ACCESS AND PAYMENT FOR MEDICAL CARE SERVICES DELIVERED TO INDIVIDUALS WITHIN OUR COMMUNITY. WE WORK COLLABORATIVELY WITH PAYERS, PROVIDERS AND EMPLOYEES TO ORGANIZE EDUCATION PROGRAMS FOR AREA HEALTH CARE PROVIDERS, EDUCATE LOCAL EMPLOYERS REGARDING EFFICIENT AND AFFORDABLE HEALTH CARE, AND PARTICIPATE WITH OTHER AREA ORGANIZATIONS IN HEALTH EDUCATION AND CLINICAL INTEGRATION INITIATIVES. MORE SPECIFICALLY, THE FOLLOWING PROVIDER SERVICES ARE AVAILABLE TO ALL NETWORK MEMBERS: EDUCATION ON PAYER CONTRACTS, CLAIMS RESOLUTION, CLINICAL EDUCATION INCLUDING CME/CEU, PROVIDER-TO-PROVIDER NETWORKING, CREDENTIALING, PHYSICIAN ACCESS, OUTREACH, RECRUITMENT, SUPPORT OF HEALTH INFORMATION EXCHANGE AND PORTAL USE FOR COORDINATION OF CARE AND ENHANCED PROVIDER COMMUNICATION, AND COMPREHENSIVE REPORTING STRATEGY FOR UTILIZATION, RISK, QUALITY AND INCENTIVES. WE SUPPORT PROCESS IMPROVEMENT OF CLINICAL INTEGRATION MODELS, INCLUDING THE CHRONIC CARE MODEL TO IMPROVE CARE OF CHRONIC DISEASE, AND PATIENT CENTERED MEDICAL HOME MODELS OF MULTI-DISCIPLINARY TEAMS FOR PRIMARY CARE DELIVERY. WE ALSO OFFER THE FOLLOWING INFORMATION TECHNOLOGY SUPPORT: POPULATION HEALTH MANAGEMENT, DATA ANALYSIS AND REPORT GENERATION, PAYER REGISTRY SUPPORT SERVICES, AND ANALYSIS OF FEE SCHEDULES.
