THE COMMUNITY HEALTH INNOVATION REGION PROJECT IS AN EFFORT TO HELP THE STATE AND FEDERAL GOVERNMENT IMPLEMENT INNOVATIONS INTENDED TO ADDRESS THE TRIPLE AIM OF BETTER HEALTH, BETTER CARE AND LOWER COST. IT DOES THIS THROUGH PROJECTS INTENDED TO HELP VULNERABLE PEOPLE GET THE RIGHT CARE AT THE RIGHT PLACE AT THE RIGHT TIME AND THROUGH PROJECTS INTENDED TO ADDRESS SOCIAL DETERMINATES OF HEALTH.
