THIS DEVELOPMENT PROVIDES 16 UNITS OF SAFE, AFFORDABLE HOUSING LINKED TO RENTAL SUBSIDIES AND SERVICES FOR LOW-INCOME PERSONS DISABLED WITH MENTAL ILLNESS. THE SITE ALSO INCLUDES A COMMUNITY BUILDING AND LAUNDRY. IN ADDITION TO PERFORMING ALL CUSTOMARY PROPERTY MANAGEMENT SERVICES, CHN ALSO OFFERS MANY DIFFERENT SUPPORTIVE SERVICES TO RESIDENTS AND EMPLOYS A PART-TIME RESIDENT MANAGER TO BE ON-SITE. CHN STAFF HELP RESIDENTS NAVIGATE THE HOUSING'S MANY FUNDING COMPLIANCE REQUIREMENTS; EVALUATE THEIR SERVICE AND HOUSING NEEDS; REGULARLY VISIT THEM TO MONITOR THEIR WELL-BEING; AND LINK THEM TO THE APPROPRIATE SERVICE PROVIDERS FOR SPECIFIC NEEDS, SUCH AS TRANSPORTATION ASSISTANCE, LIFE SKILLS ASSISTANCE, HEALTH AND WELLNESS EDUCATION, FINANCIAL LITERACY, AND EMPLOYMENT ASSISTANCE. CHN ALSO COORDINATES WITH THE RESIDENT'S CASE MANAGERS TO ADDRESS ISSUES AND ADVOCATE FOR ADDITIONAL SERVICES AS NEEDED TO SUPPORT HOUSING RETENTION AND IMPROVED OUTCOMES, WHICH RESULTS IN A MORE EFFECTIVE AND EFFICIENT SYSTEM OF CARE.
