GREATER FAMILY HEALTH IS A NONPROFIT CORPORATION PROVIDING ACCESS TO QUALITY HEALTHCARE FOR THE UNINSURED AND UNDERSERVED RESIDENTS OF THE GREATER ELGIN, MCHENRY, STREAMWOOD, DEKALB, SYCAMORE, WHEELING, AND HANOVER PARK COMMUNITIES.

GREATER FAMILY HEALTH IS A JOINT COMMISSION-ACCREDITED, PRIMARY CARE MEDICAL HOME-CERTIFIED FEDERALLY QUALIFIED HEALTH CENTER PROVIDING COMPREHENSIVE ACCESS TO HIGH-QUALITY, AFFORDABLE AND INTEGRATED HEALTH CARE IN THE SPECIALTIES OF FAMILY PRACTICE, PEDIATRICS, OBSTETRICS & GYNECOLOGY, ORAL HEALTH AND BEHAVIORAL HEALTH. THROUGH 11 HEALTH CENTERS LOCATED IN THE COMMUNITIES OF DEKALB, ELGIN, FRANKLIN PARK, HANOVER PARK, MCHENRY, PALATINE, STREAMWOOD, SYCAMORE AND WHEELING, GREATER FAMILY HEALTH WAS ABLE TO PROVIDE CARE TO 58,831 PATIENTS THROUGH 189,026 VISITS DURING THE 12-MONTH PERIOD ENDING JUNE 30, 2022. FURTHERMORE, GREATER FAMILY HEALTH HAS AGAIN BEEN RECOGNIZED BY THE US DEPARTMENT OF HEALTH AND HUMAN SERVICES AS A NATIONAL QUALITY LEADER, WITH OUR CY 2021 CLINICAL OUTCOMES RANKED IN THE TOP 10% NATIONALLY. GREATER FAMILY HEALTH ALSO REMAINS A 2017 MILLION HEARTS HYPERTENSION CONTROL CHAMPTION, A 5-YEAR HONOR, AWARDED BY THE CENTERS FOR DISEASE CONTROL AND PRVENTION.
