GREATER FAMILY HEALTH, FORMERLY KNOWN AS GREATER ELGIN FAMILY CARE CENTER, IS A JOINT COMMISSION-ACCREDITED, PRIMARY CARE MEDICAL HOME-CERTIFIED FEDERALLY QUALIFIED HEALTH CENTER PROVIDING COMPREHENSIVE ACCESS TO HIGH-QUALITY, AFFORDABLE AND INTEGRATED HEALTH CARE IN THE SPECIALTIES OF FAMILY PRACTICE, PEDIATRICS, OBSTETRICS & GYNECOLOGY, ORAL HEALTH AND BEHAVIORAL HEALTH, INCLUDING SUBSTANCE USE DISORDER SERVICES. THROUGH 9 HEALTH CENTERS LOCATED IN THE COMMUNITIES OF DEKALB, ELGIN, HANOVER PARK, MCHENRY, STREAMWOOD, SYCAMORE AND WHEELING, GREATER FAMILY HEALTH WAS ABLE TO PROVIDE CARE TO 62,585 PATIENTS THROUGH 179,676 VISITS DURING THE 12-MONTH PERIOD ENDING JUNE 30, 2021. GREATER FAMILY HEALTH IS ALSO A 2017 MILLION HEARTS HYPERTENSION CONTROL CHAMPION, A 5-YEAR HONOR, AWARDED BY THE CENTERS FOR DISEASE CONTROL AND PREVENTION.
