PREVENTION SERVICES- RELATE TO THE PREVENTION OF CHRONIC KIDNEY DISEASES, PRIMARILY THROUGH THE INDIANA KIDNEY CHECK SCREENING PROGRAM. OVER 150 INDIVIDUALS WERE SERVICED FOR THE FISCAL YEAR ENDED JUNE 30, 2021.
PUBLIC HEALTH EDUCATION- CONSISTS OF WORKSHOPS, HEALTH FAIRS, BROCHURES, VIDEO PRODUCTION, OR ANY OTHER EDUCATIONAL PROJECT OR PROGRAM TO EDUCATE THE PUBLIC ON KIDNEY DISEASES, UROLOGICAL DISEASES, DIABETES, HYPERTENSION, ORGAN AND TISSUE DONATION OR TRANSPLANTATION.
PATIENT SERVICES- PROVIDE AID TO KIDNEY DISEASE AND TRANSPLANT PATIENTS INCLUDING THE NATIONAL KIDNEY FOUNDATION SCHOLARSHIP PROGRAM, ADVOCACY, EMERGENCY SERVICES, AND SUMMER CAMP.
