MERCY LIFE OF ALABAMA IS A PACE PROGRAM, PROGRAM OF ALL-INCLUSIVE CARE FOR THE ELDERLY. IT WAS FORMED IN 2011 AS THE FIRST PACE ORGANIZATION IN THE STATE OF ALABAMA, AND SERVES THE RESIDENTS OF MOBILE AND BALDWIN COUNTIES. IT IS A UNIQUE PROGRAM PROVIDING A FULL SPECTRUM OF HEALTH CARE SERVICES, FROM PRIMARY TO ACUTE TO LONG-TERM CARE FOR FRAIL ELDERLY INDIVIDUALS WHO ARE CERTIFIED TO REQUIRE NURSING HOME CARE. THE PROGRAM IS ADMINISTERED FROM A LIFE HEALTH CENTER. AT THE CENTER, CLINICAL AND REHABILITATIVE SERVICES ARE COORDINATED AND PARTICIPANTS VISIT, SOCIALIZE AND BUILD MEANINGFUL RELATIONSHIPS WITH DOCTORS, NURSES AND OTHER PROGRAM PARTICIPANTS. THE PROGRAM IS ABLE TO PROVIDE THE ENTIRE CONTINUUM OF CARE AND SERVICES TO SENIORS WITH CHRONIC CARE NEEDS WHILE MAINTAINING THEIR INDEPENDENCE IN THEIR HOMES FOR AS LONG AS POSSIBLE.
