HOPES CENTER HELPS PEOPLE TRANSITION OUT OF HOMELESSNESS THROUGH THREE MAIN PROGRAMS: 1) STREET OUTREACH: TEAMS GO OUT TO MEET PEOPLE WHO ARE UNSHELTERED WHERE THEY ARE AND PROVIDE FOOD, BLANKETS, AND LINKAGES TO SHELTER HOUSING AND OTHER SERVICES. 2) PATH (PROJECTS FOR ASSISTANCE IN TRANSITION FROM HOMELESSNESS): A CASE MANAGER WORKS WITH PEOPLE WHO ARE HOMELESS OR AT RISK OF HOMELESSNESS AND WHO HAVE A MENTAL HEALTH CONDITION AND CONNECT THEM TO CRITICAL SERVICES SUCH AS HEALTH CARE, MENTAL HEALTH SERVICES, INCOME, AND HOUSING. 3) RAPID REHOUSING: HOPES PROVIDES HOUSING ASSISTANCE AND HOUSING STABILITY CASE MANAGEMENT TO PEOPLE EXPERIENCING HOMELESSNESS IN EMERGENCY SHELTERS OR ON THE STREETS. HOPES CENTER'S SERVICES ARE FUNDED THROUGH PRIVATE AND GOVERNMENT GRANTS, CHARITABLE CONTRIBUTIONS, AND FUNDRAISING EVENTS.
