HOPES CENTER HELPS PEOPLE TRANSITION OUT OF HOMELESSNESS THROUGH THREEMAIN PROGRAMS: 1) STREET OUTREACH: TEAMS GO OUT TO MEET PEOPLE WHO AREUNSHELTERED WHERE THEY ARE AND PROVIDE FOOD, BLANKETS, AND LINKAGES TOSHELTER HOUSING AND OTHER SERVICES. 2) PATH (PROJECTS FOR ASSISTANCE IN TRANSITION FROM HOMELESSNESS): A CASE MANAGER WORKS WITH PEOPLE WHO ARE HOMELESS OR AT RISK OF HOMELESSNESS AND WHO HAVE A MENTAL HEATH CONDITION AND CONNECT THEM TO CRITICAL SERVICES SUCH AS HEALTH CARE, MENTAL HEALTH SERVICES, INCOME, AND HOUSING. 3) RAPID REHOUSING: HOPES PROVIDES HOUSING ASSISTANCE AND HOUSING STABILITY CASE MANAGEMENT TO PEOPLE EXPERIENCING HOMELESSNESS IN EMERGENCY SHELTERS OR ON THE STREETS.
