CHF PROVIDES PERMANENT AND TRANSITIONAL HOUSING FOR PEOPLE WHO HAVE EXPERIENCED HOMELESSNESS AT FOUR PROPERTIES INCLUDING VAN BUREN HOUSE, PARTNERS PLACE, DEDES HOME, AND THIRD STREET COMMONS, FOR A TOTAL OF SIXTY EIGHT BED UNITS. RESIDENTS PAY RENT BASED ON THEIR INCOME, RENT DOES NOT EXCEED 30 PERCENT OF INCOME LEVELS. LAST FISCAL YEAR WE SERVED 41 PEOPLE IN HOUSING.
CHF CASE MANAGEMENT SERVICES PROVIDES SUPPORT TO PEOPLE IN HOUSING AND IN OUTREACH, INCLUDING INDIVIDUALS AT LOCAL SHELTERS AND PEOPLE LIVING ON THE STREET. THESE SERVICES INCLUDE ASSISTANCE WITH FINDING AND STAYING STABLE IN HOUSING, ASSISTANCE IN INCREASING SELF-SUFFICIENCY AND WELL-BEING, INCLUDING PHYSICAL AND MENTAL HEALTH, SOCIAL CONNECTIONS, AND SAFETY. CHF CASE MANAGEMENT STAFF SERVED AT LEAST 45 PEOPLE IN HOUSING AND 100 PEOPLE IN OUTREACH SERVICES.
